What Does Medicare Cost in 2026? Every Part, Explained
The standard Part B premium is $202.90 a month in 2026 and a hospital stay starts with a $1,736 deductible. Here is every 2026 Medicare cost in one place, with what it means for you in Colorado.
Key takeaways
- Most people pay no Part A premium. If you do not qualify for premium-free Part A, it costs up to $565 a month in 2026.
- A hospital stay under Original Medicare starts with a $1,736 deductible per benefit period, then $434 a day for days 61 to 90.
- The standard Part B premium is $202.90 a month and the Part B deductible is $283. Higher incomes pay more through IRMAA.
- Covered Part D drug costs are capped at $2,100 for 2026. The CMS 2027 plan files list $2,400 for 2027.
- CMS usually announces the 2027 Part A and Part B amounts in November. These are the figures that apply through December 31, 2026.
Medicare is not one bill. It is a set of premiums, deductibles and daily charges that depend on which parts you have and how you use them. In 2026 the standard Part B premium is $202.90 a month, the Part B deductible is $283, and a hospital stay begins with a $1,736 Part A deductible.
These amounts are set nationally by the Centers for Medicare & Medicaid Services (CMS), so they are the same in Brighton, Denver or anywhere else in Colorado. What changes from person to person is income, work history, and the coverage you add on top of Original Medicare.
Lisa Asmussen posted a short version of this list earlier in the year. This is the full version: every 2026 cost, what triggers it, and how a Medicare Supplement, a Medicare Advantage plan or a Part D plan changes what you actually pay. The goal is the same as always: take the stress out of Medicare.
What does Part A cost in 2026?
For most people Part A has no monthly premium, because they or a spouse paid Medicare taxes for at least 40 quarters (about 10 years) of work. If you have fewer than 30 quarters, the premium is $565 a month in 2026. With 30 to 39 quarters it is $311 a month.
The bigger Part A numbers show up when you use it. Part A is charged per benefit period, which starts the day you are admitted as an inpatient and ends after you have been out of the hospital or a skilled nursing facility for 60 days in a row. A second admission after that gap starts a new benefit period and a new deductible.
| What you use | What you pay in 2026 |
|---|---|
| Monthly premium, fewer than 30 work quarters | $565 |
| Monthly premium, 30 to 39 work quarters | $311 |
| Hospital deductible, each benefit period | $1,736 |
| Hospital days 1 to 60 | $0 after the deductible |
| Hospital days 61 to 90 | $434 a day |
| Lifetime reserve days (up to 60 in your lifetime) | $868 a day |
| Hospital days after your reserve days are used | All costs |
| Skilled nursing facility days 1 to 20 | $0 |
| Skilled nursing facility days 21 to 100 | $217 a day |
| Skilled nursing facility after day 100 | All costs |
What does Part B cost in 2026?
Most people pay the standard Part B premium of $202.90 a month in 2026, and the yearly Part B deductible is $283. After the deductible, Original Medicare generally pays 80% of the approved amount for doctor visits, outpatient care, lab work and equipment, and you pay 20% with no yearly limit.
Social Security tells you your exact amount, and it is usually taken out of your monthly benefit. According to CMS, you pay the standard amount if you are signing up for Part B for the first time in 2026, do not get Social Security benefits, are billed directly for Part B, or have both Medicare and Medicaid with Medicaid paying your premium. In Colorado, Medicaid is Health First Colorado.
- standard Part B premium per month, 2026
- $202.90
- Part B deductible for the year, 2026
- $283
- your share of most Part B services after the deductible
- 20%
- on the 20% under Original Medicare alone
- No cap
That open-ended 20% is why most people add something. A Medicare Supplement (Medigap) plan pays some or all of it, and a Medicare Advantage plan replaces it with copays and a yearly in-network out-of-pocket maximum.
Will I pay more for Part B or Part D because of my income?
Yes, if your modified adjusted gross income from your 2024 tax return was above $109,000 single or $218,000 married filing jointly. That surcharge is called IRMAA, the income-related monthly adjustment amount. It raises your Part B premium and adds an amount to whatever your Part D or Medicare Advantage drug plan charges.
| Individual tax return | Joint tax return | Part B premium | Part D IRMAA |
|---|---|---|---|
| $109,000 or less | $218,000 or less | $202.90 | plus $0 |
| Above $109,000 up to $137,000 | Above $218,000 up to $274,000 | $284.10 | plus $14.50 |
| Above $137,000 up to $171,000 | Above $274,000 up to $342,000 | $405.80 | plus $37.50 |
| Above $171,000 up to $205,000 | Above $342,000 up to $410,000 | $527.50 | plus $60.40 |
| Above $205,000 up to $500,000 | Above $410,000 up to $750,000 | $649.20 | plus $83.30 |
| Above $500,000 | Above $750,000 | $689.90 | plus $91 |

What does Part D drug coverage cost in 2026?
Each Part D plan sets its own premium, deductible and copays, but the rules around them are national. For 2026, covered Part D drug costs are capped at $2,100 for the year. Once you reach it, you pay $0 for covered drugs until January. The national base beneficiary premium, which CMS uses to figure the late enrollment penalty, is $38.99 a month.
| Measure | 2026 | 2027 |
|---|---|---|
| Out-of-pocket cap on covered drugs | $2,100 | $2,400 |
| Highest deductible a Colorado stand-alone plan charges | $615 | $700 |
| Stand-alone Part D plans offered in Colorado | 10 | 8 |
| Covered insulin, one month | $35 or less | $35 or less |
Waiting to join a drug plan can cost you for life. The late enrollment penalty adds 1% of the base premium for every full month you went without creditable drug coverage. Twelve months late adds $4.70 a month in 2026, and it is recalculated each year as the base premium changes. More on 2027 drug plans in Part D planning for 2027 in Colorado.
How does extra coverage change what I pay?
The numbers above are Original Medicare by itself. Most people in Colorado add one of two paths, and each one reshapes the bills in a different way.
Original Medicare plus a Medigap plan
- You pay the Part B premium plus the Medigap premium
- Plan G covers the Part A deductible and coinsurance and the 20% Part B share, leaving the $283 Part B deductible
- You add a stand-alone Part D plan for prescriptions
- Any doctor in the U.S. who accepts Medicare
A Medicare Advantage plan
- You still pay the Part B premium, plus any plan premium
- Copays replace the Part A deductible and the 20%
- A yearly in-network out-of-pocket maximum caps medical costs
- Most plans include drug coverage and use a network
Neither path is right for everyone. A Medigap plan costs more each month but makes a hospital year predictable. A Medicare Advantage plan can cost less month to month, but a hard year can reach the plan's maximum. For 2027 plans in Adams County, see the out-of-pocket maximum range.

What should I do before the 2027 amounts come out?
Use the 2026 figures for this year and plan for change. CMS usually announces the 2027 Part A and Part B amounts in November, after Medicare open enrollment has started, so your 2027 plan choice has to be made with the 2027 plan premiums you can see now and the 2026 federal amounts as a guide.
- Check your Social Security statement each January for the new Part B amount.
- Look at your 2024 tax return if you are near an IRMAA bracket, and file SSA-44 if your income has dropped since.
- Read your plan's Annual Notice of Change for 2027 premiums, deductibles and the out-of-pocket maximum.
- List every prescription so a drug plan comparison uses your real medicines and pharmacy.
- Compare during open enrollment, October 15 to December 7, 2026, for coverage that starts January 1, 2027.
The bottom line
In 2026 Medicare costs most people $202.90 a month for Part B, with a $283 Part B deductible, a $1,736 Part A deductible per hospital benefit period, and a $2,100 yearly cap on covered drug costs. The extra coverage you choose decides how much of the rest lands on you. Check your income bracket, read your 2027 plan letters, and compare before December 7.
