Dacono at a glance
The Census Bureau counts about 6,500 Dacono residents, 10.5% of them 65 or older, with a median household income of about $104,600. CDC PLACES estimates show obesity at 33.6%, above the Colorado county average.
Dacono is in Weld County, so residents choose from the Weld County plan list for 2027. The nearest hospitals in the CMS file are across the county line in Longmont. A PPO lets you use doctors outside the plan network at a higher cost, which can matter when your doctors are in a different county from your home, so the PPO plans are shown first.
How Lisa helps here. Dacono clients work with Lisa by phone or video, or at the Brighton office. She checks that your Longmont or Boulder County doctors are in network before recommending any plan.
10.5% of Dacono residents are 65 or older, against 15.6% for Colorado, and 2.9% have no health insurance.

How did Dacono get its name, and what built the town?
Dacono was first settled in 1901 and incorporated on January 23, 1908. Charles Lockhard Baum, one of the three people who filed the papers, named it from the first two letters of three women's names: Daisy, Cora Van Voorhies and Nora Brooks. The city notes that the Rocky Mountain News once called it the most unique name in the state.
Coal built the town. Settlers came from Greece, France, Bulgaria and Italy to work the mines, which ran from 1900 to the late 1970s, and a rail line carried coal and produce to Denver. When oil and natural gas were found in the early 1970s, coal declined quickly. Farms around the city raised corn, grains, onions, pinto beans, potatoes, sugar beets, tomatoes and wheat.
I have retiree coverage from my old job. Do I still need Medicare?
Usually yes, and on time. Mining, energy and other long careers sometimes end with retiree health coverage, and that is where people get caught. Medicare.gov explains that retiree coverage is not the same as coverage from a job you still hold: with retiree coverage, the safe move is to enroll in Medicare at 65, because waiting can bring a monthly late enrollment penalty.
Medicare.gov adds that you may need both Part A and Part B to get full benefits from retiree coverage, and that retiree coverage might not pay during any time you were eligible for Medicare but did not sign up. Ask the plan administrator, in writing if you can, how it works with Medicare and whether its drug coverage is creditable. Lisa reviews that letter with Dacono clients by phone or video before anyone drops or changes coverage.
Can I get a ride from Dacono to a doctor in Longmont or Boulder County?
Yes, if you qualify for the City of Dacono's ride program. Dacono funds Via paratransit for three groups: people aged 60 and up, adults with a disability, and households earning no more than 80% of the area median income for Weld County. Register with Via first by calling (303) 444-3043. Weekday rides operate between 8:00 a.m. and 3:30 p.m. inside Dacono and between Dacono and Firestone, Frederick, Erie or Boulder County, though trips between communities may be limited to certain days.
60+ Ride is the other local option the city lists: no-cost rides for anyone 60 or older living in Weld County. Call (970) 352-9348; signing up costs nothing too.
Those routes line up with where many Dacono patients already go, which brings the network question back. Your choices come from Weld County, but your doctor may be in Longmont or Boulder County. A PPO lets you see doctors outside its network at a higher cost; an HMO generally covers only emergency and out-of-area urgent care outside the network. Before you book a ride across the county line, make sure that doctor takes your plan.
Where does a Dacono household apply for Health First Colorado or Medicare Savings help?
Through Weld County Human Services. You can apply online through Colorado PEAK, by phone at (970) 400-6012, or in person. The closest county office to Dacono is the Del Camino (Southwest) office at 4209 County Road 24 1/2, open Wednesdays only from 8:00 a.m. to noon and 1:00 to 5:00 p.m.; the Fort Lupton and Greeley offices are open weekdays.
Health First Colorado (Colorado Medicaid) covers eligible adults and children, and the Medicare Savings Programs can pay some or all of the Medicare premiums for people with limited income and resources. Your county of residence handles both applications. If you qualify for either, it changes which Medicare plan fits, so it is worth checking before you compare plans.

CDC PLACES: Local Data for Better Health, Place Data, 2025 release (2023 survey estimates), retrieved 2026-10-14. Model-based small-area estimates of crude prevalence among adults 18 and over, not a count of local diagnoses. Crude rates follow the age mix of a place: an older community reads higher on most conditions. Colorado figure: average of the 64 Colorado counties, weighted by adult population.
Why this matters for plan choice
Dacono runs above the Colorado county average on diagnosed diabetes. The chronic-condition Special Needs Plans on the Weld County list are built around: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus. If you live with one of those, a C-SNP may be worth a look; you need a qualifying diagnosis to join.
Weld County plans: 2026 to 2027
Your plan mailed an Annual Notice of Change by September 30, 2026. It lists what changes on January 1, 2027: premium, deductible, out-of-pocket limit, copays, drug list and network. Compare it line by line, or bring it to Lisa.
Counting plans with drug coverage, Special Needs Plans included, the Weld County list goes from 53 plans in 2026 to 42 in 2027, and the average in-network out-of-pocket limit moves from $6,757 to $7,710.
| Measure | 2026 | 2027 |
|---|---|---|
| Plans with drug coverage (SNPs included) | 53 | 42 |
| With a $0 combined monthly premium for Parts C and D | 28 | 28 |
| Average in-network out-of-pocket limit | $6,757 | $7,710 |
| Average Part D deductible | $436.42 | $533.21 |
| Maximum Part D deductible allowed by CMS | $615 | $700 |
| Part D out-of-pocket cap | $2,100 | $2,400 |
These counts cover plans with drug coverage, Special Needs Plans included, so they differ from the headline count of 25, which leaves out Special Needs Plans and includes plans without drug coverage. Clear Spring Health has no Medicare Advantage plan with drug coverage in Weld County for 2027.
13 plans on the 2026 Weld County list end with no similar plan in the county, including Select Health Medicare + Kroger (HMO), Select Health Medicare Flex (HMO), Select Health Medicare Active (HMO). 4 more plans end while the same company offers a plan with the same or a similar name under a new plan ID (HealthSpring TotalCare (HMO D-SNP), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring Preferred (HMO) and Select Health Medicare Essential (HMO)). Members are not moved into the new plan automatically; to keep it, enroll in the new plan ID by December 7, 2026. The full list is on the Weld County guide.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.

How the figures are derived
- Headline plan counts and averages exclude Special Needs Plans, which have their own eligibility rules and are reported separately.
- "$0 Part C premium" counts plans whose Part C premium is $0. "$0 combined monthly premium for Parts C and D" counts plans whose consolidated Part C + D premium is $0 (for a plan without drug coverage, its Part C premium); our guides and blog use this second count. Out-of-pocket limits are in-network amounts. The average Part D deductible covers plans with drug coverage only.
- The 2026 to 2027 comparison counts Medicare Advantage plans with drug coverage, Special Needs Plans included, and its "$0" row uses the combined Part C + D premium, so it differs from the headline count.
- When the CMS crosswalk ends a plan with no successor but the same company files a plan with the same or a similar name under a new plan ID, we say so; members are not moved into the new ID automatically.
- We do not publish per-plan enrollment: the source does not carry it, and we never estimate plan figures.
