The community

Fort Lupton at a glance

The Census Bureau counts about 8,900 Fort Lupton residents, 11.3% of them 65 or older, with a median household income of about $80,900. 13.0% of residents have no health insurance. In the CDC PLACES estimates, Fort Lupton adults run well above the Colorado county average on diabetes (12.7%), high blood pressure (30.5%) and COPD (6.7%).

Fort Lupton is in Weld County, so residents choose from the Weld County plan list, not the Adams County list that Brighton uses. Weld County has fewer plans for 2027. Chronic condition Special Needs Plans are on the Weld list too, built around heart conditions and diabetes; they are shown below because they match the local health picture. The nearest hospitals in the CMS file are in Brighton and Greeley.

How did Fort Lupton go from a fur post to a farm town?

The name is older than the city. In 1836 Lancaster Lupton built an adobe trading post on the South Platte, first called Fort Lancaster, and traded with the Cheyenne, the Arapaho and the mountain men. The fur trade faded in the 1840s and the post was abandoned. The town itself came later: in 1881 William G. Winbourn, who had persuaded the railroad to run a sidetrack to his land, founded it about a mile south of the old fort.

What followed was a farm and food processing economy. Weld County's own history lists cattle, sheep and poultry, vegetable canning and shipping companies, a sugar processing factory, a condensed milk plant and feed businesses. The Fort Lupton City Museum, housed in the 1929 library building designed by John J. Huddart, keeps that story, down to the canning factory known for its tomatoes.

Farm and plant work often runs by the season, and that matters for coverage before 65. Financial help on Connect for Health Colorado and eligibility for Health First Colorado (Colorado Medicaid) both depend on household income, so a year with a big change in income is a year to check again rather than renew on autopilot.

Where can I see a doctor and get enrollment help without leaving Fort Lupton?

Salud Family Health, a community health center, has its administrative office in Fort Lupton at 203 S. Rollie Avenue and a clinic at 1115 2nd Street. The clinic lists medical and behavioral health care Monday to Friday, 7:30 a.m. to 5:00 p.m., dental care from 8:00 a.m. to 5:00 p.m., and a Nurse Advice Line after hours at (303) 697-2583. Salud offers a sliding fee scale based on family size and income.

Salud also has enrollment specialists. Its health coverage page lists help with Health First Colorado, Child Health Plan Plus (CHP+), private plans through Connect for Health Colorado or OmniSalud, the Medicare Savings Programs, and the Old Age Pension program for low-income Coloradans 60 and older.

Two insurance points follow from that. If Salud is where you get your care, check that the clinic is in the network of any Medicare Advantage plan before you switch to it. And if your income is limited, ask about a Medicare Savings Program before you pick a plan, because it can pay your Part B premium and change which plan makes sense. Lisa can do that check with you at the Brighton office, a short drive south, or by phone or video.

What do the Recreation Center senior programs cover that Medicare does not?

The Active Adults program at the Fort Lupton Recreation Center, 203 S. Harrison Avenue, is the town's gathering place for older residents. The city lists these regular services:

  • Friendly Fork senior lunch on Mondays at noon through the Weld County Area Agency on Aging. Reserve by the Thursday before at (303) 857-4200; the city lists a suggested donation of $4.00 for seniors 60 and older.
  • Hearing clinics on the first Monday at the senior lunch.
  • Blood pressure checks on the second and fourth Mondays by the local fire department and paramedics.
  • A foot care clinic on the fourth Wednesday of each month, hosted by the Nurse Service Association of Colorado. Appointments: (303) 857-4200 ext. 6166.
  • Rides through Via Mobility for seniors 60 and over or people unable to drive, Monday to Friday, 8:00 a.m. to 3:30 p.m., at (303) 444-3043.

Why do those hearing and foot clinics matter when I compare plans?

Because they point to two gaps in Original Medicare. Medicare.gov is plain that Medicare does not cover hearing aids or exams for fitting them. Routine foot care, such as trimming nails or treating corns and calluses, is generally not covered either; Medicare covers podiatrist care for medically necessary treatment of foot injuries or diseases, and routine care only in very limited circumstances.

Some Medicare Advantage plans add hearing benefits that Original Medicare does not include, and each plan sets its own rules, allowances and network for them. If you already rely on the clinics at the Recreation Center, or you know a hearing aid is coming, put that on the list next to your doctors and prescriptions. Then the comparison is about the whole year of care, not only the monthly premium.

For anything the Recreation Center and your plan do not cover, Weld County runs a Southeast Service Center in Fort Lupton. Its Assistance Payments office, (303) 857-4052, takes Medicaid applications, and the county health clinic at 2960 9th Street offers adult routine vaccines on Wednesdays and Thursdays by appointment at (970) 400-6420.

Why this matters for plan choice

Fort Lupton runs above the Colorado county average on diagnosed diabetes, high blood pressure, high cholesterol, coronary heart disease and stroke. The chronic-condition Special Needs Plans on the Weld County list are built around: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus. If you live with one of those, a C-SNP may be worth a look; you need a qualifying diagnosis to join.

13 plans on the 2026 Weld County list end with no similar plan in the county, including Select Health Medicare + Kroger (HMO), Select Health Medicare Flex (HMO), Select Health Medicare Active (HMO). 4 more plans end while the same company offers a plan with the same or a similar name under a new plan ID (HealthSpring TotalCare (HMO D-SNP), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring Preferred (HMO) and Select Health Medicare Essential (HMO)). Members are not moved into the new plan automatically; to keep it, enroll in the new plan ID by December 7, 2026. The full list is on the Weld County guide.

If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.

Are Fort Lupton Medicare plans the same as Brighton plans?

No. Fort Lupton is in Weld County and most of Brighton is in Adams County, and CMS approves Medicare Advantage plans county by county. Many carriers sell in both, but the Weld County list is shorter, so always compare the list for the county on your address.

Can someone in Fort Lupton with diabetes join a special plan?

Possibly. The chronic condition Special Needs Plans (C-SNPs) on the Weld County list are limited to people with qualifying conditions, which include diabetes, chronic heart failure and cardiovascular disorders. The CMS crosswalk shows 13 plans ending with no similar plan in the county, 4 plans ending while the same company offers a plan with the same or a similar name under a new plan ID (members must enroll in it themselves), 3 plans folding into another plan automatically, and 9 new plans. Provenance

Sources and methodology

How the figures are derived

  • Headline plan counts and averages exclude Special Needs Plans, which have their own eligibility rules and are reported separately.
  • "$0 Part C premium" counts plans whose Part C premium is $0. "$0 combined monthly premium for Parts C and D" counts plans whose consolidated Part C + D premium is $0 (for a plan without drug coverage, its Part C premium); our guides and blog use this second count. Out-of-pocket limits are in-network amounts. The average Part D deductible covers plans with drug coverage only.
  • The 2026 to 2027 comparison counts Medicare Advantage plans with drug coverage, Special Needs Plans included, and its "$0" row uses the combined Part C + D premium, so it differs from the headline count.
  • When the CMS crosswalk ends a plan with no successor but the same company files a plan with the same or a similar name under a new plan ID, we say so; members are not moved into the new ID automatically.
  • We do not publish per-plan enrollment: the source does not carry it, and we never estimate plan figures.
