D-SNP: 19 plans
For people with both Medicare and Health First Colorado (Colorado Medicaid).
Jefferson County has the oldest population of the nine counties in this guide, and the longest Medicare Advantage list. Here is every plan CMS lists for 2027.
Examples of plans with drug coverage and a $0 combined monthly premium for Parts C and D, by 2026 star rating and out-of-pocket limit
CMS Medicare Advantage and Part D Landscape, contract year 2027. A plan's fit depends on your doctors and prescriptions; Lisa checks both before you choose.
Every figure comes from the CMS Landscape file for contract year 2027. Special Needs Plans are counted separately.
Two ways to count "$0 premium" plans: 34 have a $0 Part C premium, and 31 have a $0 combined monthly premium for Parts C and D (26 of those include drug coverage; 5 have no drug coverage, so no Part D premium). The average premium figures likewise show the Part C premium and the combined Part C + D premium separately. Star ratings shown are 2026 ratings, because CMS’s 2027 ratings are not yet in our data snapshot.
PPO plans are the largest group here (14 of 37), and 5 plans have no drug coverage.

The Census Bureau counts about 579,400 Jefferson County residents, and about 103,600 of them, 17.9%, are 65 or older, the highest share of the nine counties here. The median age is 40.5.
The Jefferson County list applies to every Jefferson County address, including the western part of Westminster. With this many plans, the useful first cut is your doctors and your hospital, then the out-of-pocket limit and drug costs.
How Lisa helps here. Jefferson County clients work with Lisa by phone or video, or at the Brighton office.
17.9% of Jefferson County residents are 65 or older, against 15.6% for Colorado, and 5.3% have no health insurance.

The Denver Federal Center sits at West 6th Avenue and Kipling Street in Lakewood. GSA describes it as a campus created after World War II from the Denver Ordnance Plant, with the government keeping 690 acres for federal agencies. If you retired from a federal agency there, or anywhere else, your Medicare choices work differently from those of most retirees.
The Office of Personnel Management (OPM) advises federal annuitants to enroll in premium-free Part A, and says most federal employees and annuitants get Part A at 65 at no cost. Part B is a separate choice: everyone pays a premium for it, and OPM points you to Social Security's premium details before deciding. Your FEHB plan continues either way, and once you are retired and on Medicare, Medicare pays first and FEHB pays second.
Whether Part B is worth its premium depends on your FEHB plan, your doctors and whether you might ever want a Medicare Advantage option. Lisa can lay out the cost of each path for your household by phone or video.
Jefferson County Human Services takes applications for the Medicare Savings Program, which helps people with limited income and resources pay some or all of their Medicare premiums and can help with some deductibles and coinsurance. The county says no interview is required, and coverage can be backdated to provide three months of coverage before the application.
Apply through Colorado PEAK or at the Human Services building, 900 Jefferson County Parkway in Golden. Have proof of income and assets, your Medicare coverage, Jefferson County residence, age and lawful presence ready. Call 303-271-1388, or toll free 888-256-0321. The same office handles Old Age Pension for low-income residents 60 and older, which can include medical coverage through Health First Colorado or the Old Age Pension Health Care Program. Unlike the Medicare Savings Program, an Old Age Pension application is followed by an interview with an Intake Program Specialist, and some applicants must apply for Social Security benefits as part of it.
Jefferson County's Area Agency on Aging is the Denver Regional Council of Governments, whose State Health Insurance Assistance Program counselors serve Jefferson County residents with free Medicare counseling.
In the CMS hospital file, Jefferson County's hospitals include St. Anthony Hospital at 11600 West 2nd Place in Lakewood, OrthoColorado Hospital next door at 11650 West 2nd Place, and Intermountain Health Lutheran Hospital in Wheat Ridge. Because a Medicare Advantage network can include one system's hospitals and leave out another's, check the Lakewood and Wheat Ridge hospitals by name in each plan's directory, along with any Denver hospital your specialists use.
For trips without a car, RTD's W Line light rail runs 12.1 miles from Denver Union Station to the Jefferson County Government Center in Golden, with stops that include Federal Center and Lakewood Wadsworth. It opened on April 26, 2013. Residents 60 and older who cannot use transit can ask the Denver Regional Council of Governments about its no-cost Choice Services transportation at 303-480-6700.
Yes. Jefferson County Public Health's clinic at 645 Parfet Street in Lakewood vaccinates people of all ages, including flu and COVID-19 shots for adults, and its team can visit homes to give vaccines to people who cannot leave because of medical or mobility issues. Request a visit through the county's online interest form or call the front desk at 303-239-7040.
The clinic's insurance list names Health First Colorado and several private plans but does not say whether it bills Medicare, so ask when you call. Under Medicare, Part B covers the flu shot with nothing to pay when the provider accepts assignment, and Part D covers the shingles vaccine at no cost to you.

Colorado totals are the sum of all 64 Colorado counties in the same Census release; a statewide median is not computed here. Survey estimates for small towns carry wide margins of error.
Grouped by plan type, because type decides how the network works: HMOs expect you to stay in network, PPOs let you go outside it at a higher cost.

No plan on the Jefferson County list has a 5-star 2026 rating.
Plan choice follows the conditions you actually live with, because they decide which specialists and drugs you need covered.

CDC PLACES: Local Data for Better Health, County Data, 2025 release (2023 survey estimates), retrieved 2026-10-14. Model-based small-area estimates of crude prevalence among adults 18 and over, not a count of local diagnoses. Crude rates follow the age mix of a place: an older community reads higher on most conditions. Colorado figure: average of the 64 Colorado counties, weighted by adult population.
Jefferson County runs above the Colorado county average on high cholesterol and high blood pressure. The chronic-condition Special Needs Plans on the Jefferson County list are built around: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus. If you live with one of those, a C-SNP may be worth a look; you need a qualifying diagnosis to join.
Your plan mailed an Annual Notice of Change by September 30, 2026. It lists what changes on January 1, 2027: premium, deductible, out-of-pocket limit, copays, drug list and network. Compare it line by line, or bring it to Lisa.
Counting plans with drug coverage, Special Needs Plans included, the Jefferson County list goes from 65 plans in 2026 to 60 in 2027, and the average in-network out-of-pocket limit moves from $6,461 to $7,128.
| Measure | 2026 | 2027 |
|---|---|---|
| Plans with drug coverage (SNPs included) | 65 | 60 |
| With a $0 combined monthly premium for Parts C and D | 39 | 44 |
| Average in-network out-of-pocket limit | $6,461 | $7,128 |
| Average Part D deductible | $407.92 | $494.88 |
| Maximum Part D deductible allowed by CMS | $615 | $700 |
| Part D out-of-pocket cap | $2,100 | $2,400 |
These counts cover plans with drug coverage, Special Needs Plans included, so they differ from the headline count of 37, which leaves out Special Needs Plans and includes plans without drug coverage. Clear Spring Health has no Medicare Advantage plan with drug coverage in Jefferson County for 2027.

New to the Jefferson County list for 2027: UHC Dual Complete CO-S4 (HMO-POS D-SNP), HealthSpring Preferred (HMO), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring TotalCare (HMO D-SNP), Anthem Chronic Care (HMO-POS C-SNP), Humana Total Complete (HMO), Aetna Medicare Full Dual Extra Care (HMO D-SNP), Aetna Medicare Signature (HMO), Aetna Medicare Prime (HMO), DEVOTED C-SNP GIVEBACK EXTRAS 016 CO (HMO C-SNP), DEVOTED GIVEBACK EXTRAS 019 CO (HMO). Some of these carry the same or a similar name as a 2026 plan that ends (see the table above); they are new plan IDs, so members of the old plan must enroll in them.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.
Medicare-certified hospitals the CMS Hospital General Information file records in Jefferson County, with the CMS overall star rating. A rating is not network status: check each hospital against a plan.

An SNP is a Medicare Advantage plan limited to people who meet a condition. They are left out of the counts above because not everyone can join.
For people with both Medicare and Health First Colorado (Colorado Medicaid).
For people with qualifying chronic conditions: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus.
For people who live in, or need the level of care of, a long-term care facility.

Every Advantage plan on this page replaces Original Medicare with a plan network. The other path keeps Original Medicare and adds a Medicare Supplement (Medigap) policy plus a separate Part D plan.
Medigap has no network: any provider that accepts Medicare accepts it, anywhere in the country. Premiums are higher than most Advantage plans, and in exchange your costs are more predictable. The Colorado Division of Insurance publishes Medigap premium charts for every company selling in the state, so you can compare the same plan letter across insurers.
Timing matters. Your 6-month Medigap open enrollment window starts when you are 65 or older and enrolled in Part B. Inside it, an insurer cannot turn you down or charge more for your health. Outside it, in most cases it can. See Medigap rules in Colorado and Medicare Advantage vs Medigap.
You must have Medicare Part A and Part B to join a Medicare Advantage plan, and live in the plan's service area, here Jefferson County.
Seven months around your 65th birthday: the 3 months before, your birthday month, and the 3 months after.
October 15 to December 7, 2026. Choose or change plans for 2027; coverage starts January 1.
January 1 to March 31. If you are in a Medicare Advantage plan, you can switch plans or return to Original Medicare once.
Moving, losing employer coverage, gaining or losing Health First Colorado, or a plan not renewing can open a window outside the normal calendar.
Ways to enroll: with a licensed agent such as Lisa ((303) 887-8584), directly with a plan, or on Medicare.gov. Free counseling is also available from the Colorado SHIP at 888-696-7213. More detail: Medicare enrollment periods.
Jefferson County has 37 Medicare Advantage plans for 2027 (Special Needs Plans not included) from 10 organizations. Of those, 34 have a $0 Part C premium, and 31 have a $0 combined monthly premium for Parts C and D (26 of those include drug coverage; 5 have no drug coverage, so no Part D premium). You still pay the Part B premium, which is $202.90 a month in 2026; the 2027 amount is announced around November 2026. Source: CMS 2027 Landscape file.
Addresses in the Jefferson County part of Westminster, home to about 45,600 residents in the State Demography Office estimates. The rest of the city uses the Adams County list.
Intermountain Health Lutheran Hospital in Wheat Ridge and CommonSpirit St. Anthony Hospital in Lakewood, each rated 4 out of 5, plus OrthoColorado Hospital in Lakewood, which has no published rating. The CMS file also places UCHealth Broomfield Hospital in Jefferson County, but its Destination Drive address is in Broomfield, so it is listed on the Broomfield County guide.
Counting Medicare Advantage plans with drug coverage, Special Needs Plans included, Jefferson County goes from 65 plans in 2026 to 60 in 2027. (The headline count of 37 plans on this page leaves out Special Needs Plans and includes plans without drug coverage, which is why the numbers differ.) The CMS crosswalk shows 8 plans ending with no similar plan in the county, 5 plans ending while the same company offers a plan with the same or a similar name under a new plan ID (members must enroll in it themselves), 3 plans folding into another plan automatically, and 11 new plans. Clear Spring Health has no Medicare Advantage plan with drug coverage in Jefferson County for 2027. Your plan’s Annual Notice of Change lists what changes for you on January 1.
Census figures (ACS 2020 to 2024 5-year estimates) show 5.3% of Jefferson County residents with no health insurance, against 7.7% for Colorado. People under 65 can shop for coverage on Connect for Health Colorado, the state’s own marketplace; open enrollment for 2027 coverage starts November 1, 2026.
