D-SNP: 16 plans
For people with both Medicare and Health First Colorado (Colorado Medicaid).
Larimer County has one of the shorter Medicare Advantage lists in this guide, and it loses plans for 2027. Here is the full list and what changes.
Examples of plans with drug coverage and a $0 combined monthly premium for Parts C and D, by 2026 star rating and out-of-pocket limit
CMS Medicare Advantage and Part D Landscape, contract year 2027. A plan's fit depends on your doctors and prescriptions; Lisa checks both before you choose.
Every figure comes from the CMS Landscape file for contract year 2027. Special Needs Plans are counted separately.
Two ways to count "$0 premium" plans: 22 have a $0 Part C premium, and 19 have a $0 combined monthly premium for Parts C and D (14 of those include drug coverage; 5 have no drug coverage, so no Part D premium). The average premium figures likewise show the Part C premium and the combined Part C + D premium separately. Star ratings shown are 2026 ratings, because CMS’s 2027 ratings are not yet in our data snapshot.
PPO plans are the largest group here (11 of 24), and 5 plans have no drug coverage.
| Plan type | Plans | $0 Part C premium | Average Part C premium | Average out-of-pocket limit | Without drug coverage |
|---|---|---|---|---|---|
| PPO | 11 | 10 | $0.82 | $7,077 | 3 |
| HMO | 7 | 7 | $0 | $6,101 | 1 |
| HMO-POS | 6 | 5 | $9.17 | $6,025 | 1 |
Your plan mailed an Annual Notice of Change by September 30, 2026. It lists what changes on January 1, 2027: premium, deductible, out-of-pocket limit, copays, drug list and network. Compare it line by line, or bring it to Lisa.
Counting plans with drug coverage, Special Needs Plans included, the Larimer County list goes from 50 plans in 2026 to 41 in 2027, and the average in-network out-of-pocket limit moves from $6,882 to $7,753.
| Measure | 2026 | 2027 |
|---|---|---|
| Plans with drug coverage (SNPs included) | 50 | 41 |
| With a $0 combined monthly premium for Parts C and D | 26 | 27 |
| Average in-network out-of-pocket limit | $6,882 | $7,753 |
| Average Part D deductible | $438.60 | $542.56 |
| Maximum Part D deductible allowed by CMS | $615 | $700 |
| Part D out-of-pocket cap | $2,100 | $2,400 |
These counts cover plans with drug coverage, Special Needs Plans included, so they differ from the headline count of 24, which leaves out Special Needs Plans and includes plans without drug coverage. Clear Spring Health has no Medicare Advantage plan with drug coverage in Larimer County for 2027.

New to the Larimer County list for 2027: UHC Dual Complete CO-S4 (HMO-POS D-SNP), Select Health Medicare Essential (HMO), HealthSpring Preferred (HMO), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring TotalCare (HMO D-SNP), Humana Total Complete (HMO), Aetna Medicare Full Dual Extra Care (HMO D-SNP), DEVOTED C-SNP GIVEBACK EXTRAS 017 CO (HMO C-SNP), DEVOTED GIVEBACK EXTRAS 020 CO (HMO). Some of these carry the same or a similar name as a 2026 plan that ends (see the table above); they are new plan IDs, so members of the old plan must enroll in them.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.
Grouped by plan type, because type decides how the network works: HMOs expect you to stay in network, PPOs let you go outside it at a higher cost.

The Census Bureau counts about 367,400 Larimer County residents, and 17.4% of them, about 63,800 people, are 65 or older.
Fort Collins, Loveland, Estes Park and the rest of the county share one plan list. The tables below show every plan for 2027, and the changes section lists the plans that end or merge on January 1.
How Lisa helps here. Larimer County clients work with Lisa by phone or video.

The Larimer County Office on Aging is the county’s Area Agency on Aging, one of 16 in Colorado, funded with federal and state money for local programs for people 60 and over. Its Aging and Disability Services line is 970-498-7750, and Larimer County Human Services keeps offices in all three of the county’s larger communities: 1501 Blue Spruce Drive in Fort Collins, 200 Peridot Avenue in Loveland, and 1601 Brodie Avenue in Estes Park. The Office on Aging says its ADRC staff help with applications for Long-Term Care Medicaid and the Medicare Savings Program, and its Ombudsman Program gives advice and advocacy about assisted living residences and nursing communities. That second service matters for planning: Medicare does not generally pay for long-term care, and Medigap policies do not cover it either, so families weighing assisted living need a separate plan for that cost.
Free plan counseling comes through the State Health Insurance Assistance Program (SHIP). For 2026 and 2027, the Colorado Division of Insurance lists UCHealth Aspen Club/Senior Services as the SHIP partner for Larimer County, at (970) 495-8558. Besides its Fort Collins office, Aspen Club lists a Loveland location at UCHealth Medical Center of the Rockies, 2500 Rocky Mountain Avenue (970.624.1860).
Lisa works with Larimer County clients by phone or video. The two kinds of help fit together: the Office on Aging handles the application if you qualify for a cost program, and Lisa compares the plans that remain on the Larimer County list.
The hospital in Estes Park is a different kind of hospital. CMS lists the facility at 555 Prospect Avenue, under the name Estes Park Medical Center, as a critical access hospital, the federal designation for small rural hospitals, rather than an acute care hospital like those in Fort Collins and Loveland. Its own website now presents it as UCHealth Estes Valley Medical Center, with primary care, urgent care and specialty clinics on site.
For an Estes Valley resident, that leads to three practical questions when comparing plans:
Look at the hospital list in the CMS file and a pattern appears. UCHealth runs Poudre Valley Hospital in Fort Collins, Medical Center of the Rockies in Loveland and the Estes Park hospital. Banner runs Banner Fort Collins Medical Center and a Loveland campus of Banner North Colorado Medical Center, the same name as Banner’s Greeley hospital on 16th Street. Fort Collins and Loveland each have a hospital from both systems.
A Medicare Advantage network may include one system, both, or only parts of each, and your own doctors may be tied to one of them. So the plan question in Larimer County is often really a system question: which one do your doctors, your specialists and your preferred hospital belong to? Start there, then compare premiums and out-of-pocket limits. With Original Medicare and a Medigap policy, you can use either system wherever Medicare is accepted, which some people value enough to pay a higher monthly premium.

Colorado totals are the sum of all 64 Colorado counties in the same Census release; a statewide median is not computed here. Survey estimates for small towns carry wide margins of error.
CMS rates each contract from 1 to 5 stars. This shows the 2026 rating for each of the 24 Larimer County plans, because CMS’s 2027 ratings are not yet in our data snapshot. New plans and some small contracts are not rated.
No plan on the Larimer County list has a 5-star 2026 rating.
Medicare-certified hospitals the CMS Hospital General Information file records in Larimer County, with the CMS overall star rating. A rating is not network status: check each hospital against a plan.
5 Medicare-certified hospitals are listed, 2 with a 5 of 5 CMS overall rating; whether each is in a plan’s network is a separate question.

Names, addresses, counties and emergency flags are shown as the CMS file records them, except rows marked "Corrected", which we checked against the hospital’s own website (listed under Sources). County pages group hospitals by the county in the CMS record.
Plan choice follows the conditions you actually live with, because they decide which specialists and drugs you need covered.

An SNP is a Medicare Advantage plan limited to people who meet a condition. They are left out of the counts above because not everyone can join.
For people with both Medicare and Health First Colorado (Colorado Medicaid).
For people with qualifying chronic conditions: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus.
For people who live in, or need the level of care of, a long-term care facility.

Every Advantage plan on this page replaces Original Medicare with a plan network. The other path keeps Original Medicare and adds a Medicare Supplement (Medigap) policy plus a separate Part D plan.
Medigap has no network: any provider that accepts Medicare accepts it, anywhere in the country. Premiums are higher than most Advantage plans, and in exchange your costs are more predictable. The Colorado Division of Insurance publishes Medigap premium charts for every company selling in the state, so you can compare the same plan letter across insurers.
Timing matters. Your 6-month Medigap open enrollment window starts when you are 65 or older and enrolled in Part B. Inside it, an insurer cannot turn you down or charge more for your health. Outside it, in most cases it can. See Medigap rules in Colorado and Medicare Advantage vs Medigap.
You must have Medicare Part A and Part B to join a Medicare Advantage plan, and live in the plan's service area, here Larimer County.
Seven months around your 65th birthday: the 3 months before, your birthday month, and the 3 months after.
October 15 to December 7, 2026. Choose or change plans for 2027; coverage starts January 1.
January 1 to March 31. If you are in a Medicare Advantage plan, you can switch plans or return to Original Medicare once.
Moving, losing employer coverage, gaining or losing Health First Colorado, or a plan not renewing can open a window outside the normal calendar.
Ways to enroll: with a licensed agent such as Lisa ((303) 887-8584), directly with a plan, or on Medicare.gov. Free counseling is also available from the Colorado SHIP at 888-696-7213. More detail: Medicare enrollment periods.
Larimer County has 24 Medicare Advantage plans for 2027 (Special Needs Plans not included) from 8 organizations. Of those, 22 have a $0 Part C premium, and 19 have a $0 combined monthly premium for Parts C and D (14 of those include drug coverage; 5 have no drug coverage, so no Part D premium). You still pay the Part B premium, which is $202.90 a month in 2026; the 2027 amount is announced around November 2026. Source: CMS 2027 Landscape file.
Yes. For 2027 the Larimer County list is shorter than the Denver County list, though the larger carriers sell in both. The numbers section on this page shows the exact count.
Poudre Valley Hospital in Fort Collins and Medical Center of the Rockies in Loveland both carry a 5 out of 5 overall rating in the CMS Hospital General Information file.
Counting Medicare Advantage plans with drug coverage, Special Needs Plans included, Larimer County goes from 50 plans in 2026 to 41 in 2027. (The headline count of 24 plans on this page leaves out Special Needs Plans and includes plans without drug coverage, which is why the numbers differ.) The CMS crosswalk shows 11 plans ending with no similar plan in the county, 4 plans ending while the same company offers a plan with the same or a similar name under a new plan ID (members must enroll in it themselves), 3 plans folding into another plan automatically, and 9 new plans. Clear Spring Health has no Medicare Advantage plan with drug coverage in Larimer County for 2027. Your plan’s Annual Notice of Change lists what changes for you on January 1.
Census figures (ACS 2020 to 2024 5-year estimates) show 5.6% of Larimer County residents with no health insurance, against 7.7% for Colorado. People under 65 can shop for coverage on Connect for Health Colorado, the state’s own marketplace; open enrollment for 2027 coverage starts November 1, 2026.
