D-SNP: 16 plans
For people with both Medicare and Health First Colorado (Colorado Medicaid).
Weld County has its own Medicare plan list, shorter than the Adams County list next door, and it shows the largest net drop in plans of any county in this guide for 2027.
Examples of plans with drug coverage and a $0 combined monthly premium for Parts C and D, by 2026 star rating and out-of-pocket limit
CMS Medicare Advantage and Part D Landscape, contract year 2027. A plan's fit depends on your doctors and prescriptions; Lisa checks both before you choose.
Every figure comes from the CMS Landscape file for contract year 2027. Special Needs Plans are counted separately.
Two ways to count "$0 premium" plans: 23 have a $0 Part C premium, and 20 have a $0 combined monthly premium for Parts C and D (15 of those include drug coverage; 5 have no drug coverage, so no Part D premium). The average premium figures likewise show the Part C premium and the combined Part C + D premium separately. Star ratings shown are 2026 ratings, because CMS’s 2027 ratings are not yet in our data snapshot.
PPO plans are the largest group here (12 of 25), and 5 plans have no drug coverage.
| Plan type | Plans | $0 Part C premium | Average Part C premium | Average out-of-pocket limit | Without drug coverage |
|---|---|---|---|---|---|
| PPO | 12 | 11 | $0.75 | $6,983 | 3 |
| HMO | 7 | 7 | $0 | $6,101 | 1 |
| HMO-POS | 6 | 5 | $9.17 | $6,025 | 1 |
Your plan mailed an Annual Notice of Change by September 30, 2026. It lists what changes on January 1, 2027: premium, deductible, out-of-pocket limit, copays, drug list and network. Compare it line by line, or bring it to Lisa.
Counting plans with drug coverage, Special Needs Plans included, the Weld County list goes from 53 plans in 2026 to 42 in 2027, and the average in-network out-of-pocket limit moves from $6,757 to $7,710.
| Measure | 2026 | 2027 |
|---|---|---|
| Plans with drug coverage (SNPs included) | 53 | 42 |
| With a $0 combined monthly premium for Parts C and D | 28 | 28 |
| Average in-network out-of-pocket limit | $6,757 | $7,710 |
| Average Part D deductible | $436.42 | $533.21 |
| Maximum Part D deductible allowed by CMS | $615 | $700 |
| Part D out-of-pocket cap | $2,100 | $2,400 |
These counts cover plans with drug coverage, Special Needs Plans included, so they differ from the headline count of 25, which leaves out Special Needs Plans and includes plans without drug coverage. Clear Spring Health has no Medicare Advantage plan with drug coverage in Weld County for 2027.

New to the Weld County list for 2027: UHC Dual Complete CO-S4 (HMO-POS D-SNP), Select Health Medicare Essential (HMO), HealthSpring Preferred (HMO), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring TotalCare (HMO D-SNP), Humana Total Complete (HMO), Aetna Medicare Full Dual Extra Care (HMO D-SNP), DEVOTED C-SNP GIVEBACK EXTRAS 017 CO (HMO C-SNP), DEVOTED GIVEBACK EXTRAS 020 CO (HMO). Some of these carry the same or a similar name as a 2026 plan that ends (see the table above); they are new plan IDs, so members of the old plan must enroll in them.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.
Grouped by plan type, because type decides how the network works: HMOs expect you to stay in network, PPOs let you go outside it at a higher cost.

CDC PLACES: Local Data for Better Health, County Data, 2025 release (2023 survey estimates), retrieved 2026-10-14. Model-based small-area estimates of crude prevalence among adults 18 and over, not a count of local diagnoses. Crude rates follow the age mix of a place: an older community reads higher on most conditions. Colorado figure: average of the 64 Colorado counties, weighted by adult population.
Weld County runs above the Colorado county average on diagnosed diabetes and high blood pressure. The chronic-condition Special Needs Plans on the Weld County list are built around: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus. If you live with one of those, a C-SNP may be worth a look; you need a qualifying diagnosis to join.
An SNP is a Medicare Advantage plan limited to people who meet a condition. They are left out of the counts above because not everyone can join.
For people with both Medicare and Health First Colorado (Colorado Medicaid).
For people with qualifying chronic conditions: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus.
For people who live in, or need the level of care of, a long-term care facility.

CMS rates each contract from 1 to 5 stars. This shows the 2026 rating for each of the 25 Weld County plans, because CMS’s 2027 ratings are not yet in our data snapshot. New plans and some small contracts are not rated.
No plan on the Weld County list has a 5-star 2026 rating.
The Census Bureau counts about 350,400 Weld County residents, 12.9% of them 65 or older. Greeley, Fort Lupton, Lochbuie, Hudson, Keenesburg, Dacono, Frederick, Firestone, Platteville and most of Erie all draw from this county plan list.
Weld County runs above the Colorado county average on several conditions in the CDC PLACES estimates, including diabetes, high blood pressure and obesity. That is one reason the Special Needs Plans section on this page deserves a careful read, not a skim.
How Lisa helps here. Southern Weld County is close to the Brighton office, and Lisa meets clients there in person, by phone or by video. Clients further north work with her by phone or video.
12.9% of Weld County residents are 65 or older, against 15.6% for Colorado, and 8.0% have no health insurance.

It does. Weld County covers 4,016 square miles, the third largest county in Colorado, and it holds 32 incorporated municipalities. Greeley, the county seat, sits in the west central part of the county, and the only two Weld County hospitals in the CMS file are both there: Banner North Colorado Medical Center and UCHealth Greeley Hospital.
That means the hospital you would actually use depends on which corner of the county you live in. In the CMS file, the nearest hospitals for Fort Lupton, Hudson and Keenesburg are in Brighton and Greeley; for Frederick, they are in Longmont; for Erie, they are in Lafayette and Longmont. In other words, many southern Weld households lean on hospitals in Adams or Boulder County, while the plan list they choose from is the Weld County list.
So before you compare premiums, write down the hospital and doctors you would really drive to. A Medicare Advantage plan sold in Weld County can still include a Brighton or Longmont hospital in its network, or it may not. That check comes first, and it is the first thing Lisa runs for a Weld County client.
The State Health Insurance Assistance Program (SHIP) is organized county by county, and the partner changes at the county line. For 2026 and 2027, the Colorado Division of Insurance lists UCHealth Aspen Club/Senior Services as the SHIP partner for Weld County, at (970) 495-8558. Next door in Adams County, the partner is a different organization, Benefits In Action. If you live in Weld County but shop or see doctors near Brighton, call the Weld County partner; your county of residence is what counts.
The Weld County Area Agency on Aging, at 315 N. 11th Ave., Building A, in Greeley, (970) 400-6950, is the place for broader help: its Options Counselor and resource line, (970) 400-6952, connects adults 60 and over with services across the county.
Southern Weld County is close to the Headwaters Health and Wealth office in Brighton, and Lisa meets clients from Fort Lupton, Lochbuie, Hudson and Keenesburg in person, as well as by phone or video. Clients in Dacono, Frederick, Firestone and towns further north work with her by phone or video, and anyone is welcome to visit the Brighton office.
Medigap policies do not cover dental, vision, hearing aids or eyeglasses, the Colorado Division of Insurance reminds shoppers, and Original Medicare leaves those costs to you as well. The Weld County Area Agency on Aging fills part of that gap locally:
Distance is a real cost in a county this size. 60+ Ride provides free, door-to-door transportation for adults 60 and older in Weld County; requests go to (970) 352-9348, Monday to Friday, 9:00 a.m. to 4:00 p.m. RideNoCo, (970) 514-3636, is a central information hub for transportation options in Northern Colorado: it points you to a ride rather than providing one.
These programs shape the insurance decision in two ways. Some Medicare Advantage plans add dental, vision, hearing or ride benefits, and others do not, so knowing what the county already offers helps you judge whether a plan extra is worth a higher premium or a narrower network. And if you count on a ride to reach a Greeley or Longmont specialist, that specialist needs to be in your plan network. Lisa lines both up before you choose.

Colorado totals are the sum of all 64 Colorado counties in the same Census release; a statewide median is not computed here. Survey estimates for small towns carry wide margins of error.
Medicare-certified hospitals the CMS Hospital General Information file records in Weld County, with the CMS overall star rating. A rating is not network status: check each hospital against a plan.
2 Medicare-certified hospitals are listed; whether each is in a plan’s network is a separate question.
| Hospital | Address | Emergency services | CMS overall rating |
|---|---|---|---|
| Banner North Colorado Medical CenterAcute Care | 1801 16th Street, Greeley, CO 80631 | Yes | 4 of 5 |
| UCHealth Greeley HospitalAcute Care | 6767 29th Street, Greeley, CO 80634 | Yes | 4 of 5 |
Names, addresses, counties and emergency flags are shown as the CMS file records them, except rows marked "Corrected", which we checked against the hospital’s own website (listed under Sources). County pages group hospitals by the county in the CMS record.
Colorado runs its own marketplace for individual and family health insurance: Connect for Health Colorado, not HealthCare.gov. Open enrollment for 2027 coverage starts November 1, 2026. Financial help depends on household income, and people with low income may qualify for Health First Colorado, the state Medicaid program, or Child Health Plan Plus (CHP+) for children.
CDC estimates that 13.7% of adults aged 18 to 64 in Weld County have no health insurance (2023 estimates), against a Colorado county average of 11.1%.
Lisa helps with Connect for Health Colorado plans and pre-65 coverage too. Try the ACA subsidy calculator, read the Connect for Health Colorado guide, or see individual and family health insurance. Estimates are estimates; final premiums are set at enrollment.
Headwaters Health and Wealth is not affiliated with, endorsed by, or operating on behalf of Connect for Health Colorado, HealthCare.gov, the Health Insurance Marketplace, or any federal or state government agency. Plan availability, premiums and advance premium tax credits are set by the carriers and the Marketplace.
Every Advantage plan on this page replaces Original Medicare with a plan network. The other path keeps Original Medicare and adds a Medicare Supplement (Medigap) policy plus a separate Part D plan.
Medigap has no network: any provider that accepts Medicare accepts it, anywhere in the country. Premiums are higher than most Advantage plans, and in exchange your costs are more predictable. The Colorado Division of Insurance publishes Medigap premium charts for every company selling in the state, so you can compare the same plan letter across insurers.
Timing matters. Your 6-month Medigap open enrollment window starts when you are 65 or older and enrolled in Part B. Inside it, an insurer cannot turn you down or charge more for your health. Outside it, in most cases it can. See Medigap rules in Colorado and Medicare Advantage vs Medigap.
