D-SNP: 19 plans
For people with both Medicare and Health First Colorado (Colorado Medicaid).
Federal Heights has the highest CDC estimate of uninsured working-age adults of any community in this guide, so coverage before 65 matters here as much as Medicare. This page covers both, starting with the help available.
Examples of plans with drug coverage and a $0 combined monthly premium for Parts C and D, by 2026 star rating and out-of-pocket limit
CMS Medicare Advantage and Part D Landscape, contract year 2027. A plan's fit depends on your doctors and prescriptions; Lisa checks both before you choose.
The Census Bureau counts about 14,100 Federal Heights residents, with a median household income of about $56,600, and 18.6% of residents have no health insurance. CDC PLACES estimates that 27.7% of adults aged 18 to 64 are uninsured, and that diabetes (12.8%), high blood pressure (30.3%) and COPD (7.4%) all run well above the Colorado county average.
Federal Heights is in Adams County, so Medicare residents choose from the Adams County plan list. People with both Medicare and Health First Colorado can look at the Dual Special Needs Plans listed below. For anyone under 65, Connect for Health Colorado and Health First Colorado are the two front doors. The nearest hospitals in the CMS file are in Thornton and Westminster.
How Lisa helps here. Federal Heights clients work with Lisa by phone or video, or at the Brighton office. She helps with Medicare, with Connect for Health Colorado plans, and with pointing you to Health First Colorado when that is the better fit.
11.6% of Federal Heights residents are 65 or older, against 15.6% for Colorado, and 18.6% have no health insurance.

Federal Heights covers about 1.78 square miles in western Adams County, and the city traces its history from early homesteads to a small town that incorporated in 1940. City Hall is at 2380 West 90th Avenue (303-412-3526).
The city's own 2021 GIS count shows how people live here: of 5,581 possible housing units, 2,433 are mobile home park spaces, about 44%, followed by 1,787 apartments and 1,113 single-family homes. The city also counts 16 assisted living units.
Housing affects Medicare in a practical way. Your plan is tied to your address, so if you move, tell Social Security and your plan right away. A move out of your plan's service area gives you a Special Enrollment Period to choose a new plan, and moving into or out of a nursing home can open choices too. Lisa can tell you which rule applies before you sign a lease.
Spanish is common here. The city's demographics page, drawing on the 2016 to 2020 American Community Survey, reports that 47.6% of residents speak Spanish at home and 50.2% speak only English.
Nearby community health centers serve Spanish speakers. Salud Family Health's enrollment specialists help with Health First Colorado, Connect for Health Colorado and OmniSalud applications (303-697-2583), and many Salud providers list Spanish among their languages. The Adams County resource guide also lists Clinica Family Health Services clinics in Thornton and Westminster.
When you compare Medicare plans, ask each one whether it provides member materials and customer service in Spanish, and whether your doctor's office speaks your language. If a family member usually translates for you, include them in the review call so nothing is lost.
Yes, and several programs stack. Salud describes the Medicare Savings Programs, which can pay some or all Medicare premiums and sometimes deductibles and coinsurance for people with limited income and resources, and Colorado's Old Age Pension, which provides financial help and health coverage to low-income residents 60 and older. Extra Help from Social Security lowers Part D drug costs.
Health First Colorado eligibility is handled by Adams County Human Services at the Pete Mirelez Human Services Center, 11860 Pecos Street in Westminster (720-523-2000), and the county resource guide notes that Benefits in Action, a nonprofit, helps with applications there. Salud's enrollment specialists also list the Medicare Savings Programs among the programs they help with.
Once a program approves you, your Medicare choices change. People with both Medicare and Health First Colorado can look at the Dual Special Needs Plans on the Adams County list, and an approval letter can also open a Special Enrollment Period.
A-Lift, the Adams County Community Transit service, takes urban Adams County residents 60 and older, and disabled residents of any age, to medical and dental appointments and grocery stores. DRCOG, the Area Agency on Aging for Adams County, also takes ride requests to the doctor's office. The nearest hospitals in the CMS file are just outside the city, in Thornton and Westminster.
Lisa Asmussen works with Federal Heights clients by phone or video, so you can review your options from home. She checks your income programs, your doctors and your prescriptions in that order, then shows you the 2027 plans that fit.

Colorado totals are the sum of all 64 Colorado counties in the same Census release; a statewide median is not computed here. Survey estimates for small towns carry wide margins of error.
Plan choice follows the conditions you actually live with, because they decide which specialists and drugs you need covered.
Compared with the Colorado county average, Federal Heights reads higher on diagnosed diabetes, high blood pressure, obesity, coronary heart disease, copd and adults under 65 without insurance.

CDC PLACES: Local Data for Better Health, Place Data, 2025 release (2023 survey estimates), retrieved 2026-10-14. Model-based small-area estimates of crude prevalence among adults 18 and over, not a count of local diagnoses. Crude rates follow the age mix of a place: an older community reads higher on most conditions. Colorado figure: average of the 64 Colorado counties, weighted by adult population.
Federal Heights runs above the Colorado county average on diagnosed diabetes, high blood pressure, coronary heart disease, high cholesterol and stroke. The chronic-condition Special Needs Plans on the Adams County list are built around: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus. If you live with one of those, a C-SNP may be worth a look; you need a qualifying diagnosis to join.
Medicare Advantage plans are approved by county, so Federal Heights residents see the Adams County list. Figures from the CMS Landscape file for contract year 2027; Special Needs Plans are counted separately.
Two ways to count "$0 premium" plans: 33 have a $0 Part C premium, and 30 have a $0 combined monthly premium for Parts C and D (25 of those include drug coverage; 5 have no drug coverage, so no Part D premium). The average premium figures likewise show the Part C premium and the combined Part C + D premium separately. Star ratings shown are 2026 ratings, because CMS’s 2027 ratings are not yet in our data snapshot.
PPO plans are the largest group here (14 of 36), and 5 plans have no drug coverage.

For people with both Medicare and Health First Colorado (Colorado Medicaid).
For people with qualifying chronic conditions: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus.
For people who live in, or need the level of care of, a long-term care facility.

Counting plans with drug coverage, Special Needs Plans included, the Adams County list goes from 64 plans in 2026 to 59 in 2027, and the average in-network out-of-pocket limit moves from $6,497 to $7,199.

8 plans on the 2026 Adams County list end with no similar plan in the county, including Select Health Medicare + Kroger (HMO), Select Health Medicare Flex (HMO), Select Health Medicare Active (HMO). 4 more plans end while the same company offers a plan with the same or a similar name under a new plan ID (HealthSpring TotalCare (HMO D-SNP), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring Preferred (HMO) and Aetna Medicare Signature (HMO-POS)). Members are not moved into the new plan automatically; to keep it, enroll in the new plan ID by December 7, 2026. The full list is on the Adams County guide.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.
New to terms like MOOP, HMO-POS or D-SNP? They are explained in plain English on the Adams County page and in the guide Medicare Advantage plan types.
