The county

Who lives in Boulder County

The Census Bureau counts about 329,000 Boulder County residents, 16.4% of them 65 or older, with a median household income of about $104,000 and 4.4% without health insurance.

Five Boulder County hospitals appear in the CMS file, in Boulder, Longmont, Lafayette and Louisville. The tables below group the county’s plans by type, with the premium, out-of-pocket limit and 2026 star rating for each.

Does Boulder County have its own Medicare counselors?

Yes. In Boulder County, the county’s own Area Agency on Aging delivers the State Health Insurance Assistance Program (SHIP): the Colorado Division of Insurance lists it as the Boulder County SHIP partner for 2026 and 2027. Its Medicare counseling team gives free, unbiased help with Part D, Medicare Advantage, Medigap, appeals, billing and claims, and coverage for people with both Medicare and Medicaid.

How to reach them: call 303-441-1546 (English and Spanish), email medicare@bouldercounty.gov, or request an appointment online. Counseling is by phone, email or appointment, and the classes are held live on Zoom: Medicare Basics sessions run through December 2026, and this fall’s open enrollment classes covered Medicare changes for 2027, how to read your Annual Notice of Change (ANOC), Extra Help, and how to avoid enrollment scams. For anyone who prefers to learn at their own pace, the county also posts a recorded three-part Medicare series, plus short Senior Medicare Patrol videos on reading your Medicare Summary Notice and spotting fraud.

Lisa is a licensed broker rather than a SHIP counselor, and she works with Boulder County clients by phone or video. A county class gives you the neutral overview; Lisa then compares the actual plans on the Boulder County list against your doctors and prescriptions and helps you enroll.

I live in the mountains above Boulder. Who helps me?

Boulder County runs Mountain Aging Services for older adults, people with disabilities, caregivers and veterans in the mountain communities. Resource specialists meet clients, learn their needs and build an action plan with them. There is a Northern Mountain Resource Specialist (720-766-8844) and a Southern Mountain Resource Specialist (720-616-9109), and the general aging line is 303-441-1617.

The county is candid about the limits: it says it generally cannot start a service where no provider exists, and that services depend on funding, staff and volunteers. That same reality shapes insurance choices in the mountains:

  • Check that the clinic you can actually reach in bad weather is in a plan’s network, not just the hospitals down in Boulder, Longmont, Lafayette and Louisville.
  • Ask how a plan handles home health and follow-up care where few providers serve.
  • Weigh the wider choice of providers that Original Medicare with a Medigap policy allows against the extra benefits a Medicare Advantage plan may add.

I am losing Health First Colorado, or I am not sure I still qualify. What now?

Boulder County’s Medicare page warns that people who recently lost Medicaid, or will soon, may be able to sign up for Medicare or change their Medicare coverage, but only for a limited time. Do not wait for the window to close.

For Health First Colorado questions, renewal notices and address updates, Boulder County Human Services is at 303-441-1000, with offices at 515 Coffman Street in Longmont, 3400 North Broadway in Boulder, and 1755 South Public Road in Lafayette. The same office can answer questions about the Medicare Savings Programs, which help pay Medicare premiums and, at the lowest income tier, deductibles and coinsurance. You can also manage your case on Colorado PEAK, where the county suggests opening your Mailbox and choosing how you want renewal notices delivered, so a letter that affects your coverage does not get lost.

The county also reminds residents that Connect for Health Colorado is not for people with Medicare, and that Medicare does not generally pay for long-term care, routine dental or eye care, dentures or hearing aids. When your Medicaid coverage changes, those gaps can appear overnight. Lisa can line up a Medicare plan that starts on time and, where it makes sense, separate dental, vision or hearing coverage.

You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.

The other path

Medigap in Boulder County

Every Advantage plan on this page replaces Original Medicare with a plan network. The other path keeps Original Medicare and adds a Medicare Supplement (Medigap) policy plus a separate Part D plan.

Medigap has no network: any provider that accepts Medicare accepts it, anywhere in the country. Premiums are higher than most Advantage plans, and in exchange your costs are more predictable. The Colorado Division of Insurance publishes Medigap premium charts for every company selling in the state, so you can compare the same plan letter across insurers.

Timing matters. Your 6-month Medigap open enrollment window starts when you are 65 or older and enrolled in Part B. Inside it, an insurer cannot turn you down or charge more for your health. Outside it, in most cases it can. See Medigap rules in Colorado and Medicare Advantage vs Medigap.

Plain English

Medicare plan terms on this page

Medicare Advantage (Part C)
A Medicare approved plan from a private company that provides your Part A and Part B benefits in place of Original Medicare. Most include drug coverage and some extras. You keep paying your Part B premium.
$0 Part C premium
The plan charges no monthly premium of its own for medical coverage. You still pay your Part B premium, any drug premium the plan lists, and copays or coinsurance when you use care.
In-network out-of-pocket maximum (MOOP)
The most you pay in a calendar year for covered Part A and Part B services from in-network providers. Once you reach it, the plan pays for covered in-network care for the rest of the year. Premiums and Part D drug costs do not count toward it.
HMO
Health Maintenance Organization. You generally use doctors and hospitals in the plan network, except for emergencies and urgent care, and many HMOs ask for a referral before you see a specialist.
PPO
Preferred Provider Organization. You can use doctors in or out of the network, but out-of-network care usually costs more. Referrals are usually not required.
HMO-POS
An HMO with a point of service option. You mostly use the network, but the plan lets you get some services out of network, usually at a higher cost.
Special Needs Plan (SNP)
A Medicare Advantage plan open only to people with particular needs: both Medicare and Medicaid (Health First Colorado in Colorado), certain chronic conditions, or living in an institution such as a nursing home.
Star rating
A yearly CMS quality score from 1 to 5 stars, based on measures such as preventive care, member experience and customer service. It describes the plan overall; it does not tell you whether your doctors or drugs are covered.

Provenance

Sources and methodology

How the figures are derived

  • Headline plan counts and averages exclude Special Needs Plans, which have their own eligibility rules and are reported separately.
  • "$0 Part C premium" counts plans whose Part C premium is $0. "$0 combined monthly premium for Parts C and D" counts plans whose consolidated Part C + D premium is $0 (for a plan without drug coverage, its Part C premium); our guides and blog use this second count. Out-of-pocket limits are in-network amounts. The average Part D deductible covers plans with drug coverage only.
  • The 2026 to 2027 comparison counts Medicare Advantage plans with drug coverage, Special Needs Plans included, and its "$0" row uses the combined Part C + D premium, so it differs from the headline count.
  • When the CMS crosswalk ends a plan with no successor but the same company files a plan with the same or a similar name under a new plan ID, we say so; members are not moved into the new ID automatically.
  • We do not publish per-plan enrollment: the source does not carry it, and we never estimate plan figures.
