D-SNP: 19 plans
For people with both Medicare and Health First Colorado (Colorado Medicaid).
Fewer than 1 in 10 Commerce City residents are 65 or older, so many households here juggle health insurance before 65 and Medicare for a parent at the same time. This page covers both.
Examples of plans with drug coverage and a $0 combined monthly premium for Parts C and D, by 2026 star rating and out-of-pocket limit
CMS Medicare Advantage and Part D Landscape, contract year 2027. A plan's fit depends on your doctors and prescriptions; Lisa checks both before you choose.
The Census Bureau counts about 66,400 Commerce City residents, with 9.7% aged 65 or older and a median age of 35.0. Median household income is about $112,000. In the CDC PLACES estimates, 19.9% of adults aged 18 to 64 have no health insurance, well above the Colorado county average of 11.1%.
Commerce City is in Adams County, so Medicare residents choose from the Adams County plan list. Some people here qualify for both Medicare and Health First Colorado, the state Medicaid program, and the Dual Special Needs Plans in the table below are built for exactly that situation. The nearest hospitals in the CMS file are in Brighton and Thornton.
How Lisa helps here. Commerce City is a short drive from the Brighton office, and Lisa meets clients there, by phone or by video. She helps with Medicare for older family members and with Connect for Health Colorado coverage for everyone else in the household.
9.7% of Commerce City residents are 65 or older, against 15.6% for Colorado, and 7.4% have no health insurance.

Commerce City runs two Active Adult Centers, both open weekdays from 8 a.m. to 4 p.m. The older one is at Eagle Pointe Recreation Center, 6060 East Parkway Drive (303-289-3756), where the city's former Senior Center was renamed in 2018 after 30 years in that building. The second is at Bison Ridge Recreation Center, 13905 East 112th Avenue (303-286-4389), in the northern part of the city. Walk-in hours are Monday mornings at Bison Ridge and Wednesday mornings at Eagle Pointe.
The city also lists a Senior Community Navigator at 720-769-5772. For residents 60 and over there is a free weekly lunch at Eagle Pointe, offered with Volunteers of America, and a GoodFriends scholarship of up to $75 a year toward program fees. A nurse holds a basic health clinic at Eagle Pointe on the first Monday of each month for blood pressure checks and foot care, by appointment at 303-698-6496.
None of that replaces a plan review, but it is a good front door. When a letter from Medicare or Health First Colorado arrives and you are not sure what it means, the navigator can point you to the right office, and Lisa can explain how it changes your coverage for 2027.
Salud Family Health runs a clinic at 6255 Quebec Parkway with medical, behavioral health and dental care and an in-clinic pharmacy. Salud is a federally funded community health center, and several of its Commerce City providers list Spanish as a language they speak. Salud also staffs a school-based site at Adams City High School, 7200 Quebec Parkway.
Salud's enrollment specialists help patients apply for Health First Colorado, CHP+, Connect for Health Colorado plans and OmniSalud, and they describe the Medicare Savings Programs that can pay Medicare premiums for people with limited income. They can be reached at 303-697-2583.
If Salud is your medical home, make it the first test for any Medicare Advantage plan: is the clinic in network for 2027, and is its pharmacy in the drug plan's network? Original Medicare with a Medigap policy works anywhere that accepts Medicare, which is a different trade-off. Lisa checks the clinic and pharmacy by name before she shows you a plan.
Commerce City is an industrial town as well as a suburb. The city notes that the Suncor refinery, just west of I-270 on Brighton Boulevard, has operated since 1931; its Plants 1 and 2 are a major supplier of gasoline and diesel fuel in Colorado, and Plant 3 is the state's main asphalt producer. Regulatory oversight sits mainly with the Colorado Department of Public Health and Environment and the EPA.
Workers from plants, rail yards and warehouses often reach 65 with a choice to make. If you are still working and covered by a large employer plan, you may be able to delay Part B without a penalty. If you are retiring with a retiree plan or a union plan, ask the plan in writing how it works with Medicare: some pay after Medicare, some require you to enroll in Part B, and dropping one can mean you cannot get it back. Self-employed tradespeople under 65 buy their own coverage through Connect for Health Colorado, where open enrollment for 2027 starts November 1, 2026.
Yes. RTD's N Line, opened September 21, 2020, stops at the Commerce City•72nd station and runs to Union Station every 20 minutes at peak times and every 30 minutes off peak. For people without a car, A-Lift, which the city lists for residents 60 or older or with a disability, covers medical and dental appointments and grocery trips on weekdays with three days' notice.
Easy access to downtown Denver is a reason to look closely at which Denver hospitals a plan includes, not only the ones in Thornton or Brighton. Lisa meets Commerce City clients at the Brighton office, a short drive up the road, or by phone or video.

Colorado totals are the sum of all 64 Colorado counties in the same Census release; a statewide median is not computed here. Survey estimates for small towns carry wide margins of error.
Medicare Advantage plans are approved by county, so Commerce City residents see the Adams County list. Figures from the CMS Landscape file for contract year 2027; Special Needs Plans are counted separately.
Two ways to count "$0 premium" plans: 33 have a $0 Part C premium, and 30 have a $0 combined monthly premium for Parts C and D (25 of those include drug coverage; 5 have no drug coverage, so no Part D premium). The average premium figures likewise show the Part C premium and the combined Part C + D premium separately. Star ratings shown are 2026 ratings, because CMS’s 2027 ratings are not yet in our data snapshot.
PPO plans are the largest group here (14 of 36), and 5 plans have no drug coverage.

For people who have both Medicare and Medicaid. In Colorado, Medicaid is Health First Colorado, and Health First Colorado decides that eligibility. From the Adams County list, CMS contract year 2027.

An SNP is a Medicare Advantage plan limited to people who meet a condition. They are left out of the counts above because not everyone can join.
For people with both Medicare and Health First Colorado (Colorado Medicaid).
For people with qualifying chronic conditions: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus.
For people who live in, or need the level of care of, a long-term care facility.
Plan choice follows the conditions you actually live with, because they decide which specialists and drugs you need covered.
Compared with the Colorado county average, Commerce City reads higher on diagnosed diabetes, obesity and adults under 65 without insurance.

CDC PLACES: Local Data for Better Health, Place Data, 2025 release (2023 survey estimates), retrieved 2026-10-14. Model-based small-area estimates of crude prevalence among adults 18 and over, not a count of local diagnoses. Crude rates follow the age mix of a place: an older community reads higher on most conditions. Colorado figure: average of the 64 Colorado counties, weighted by adult population.
Commerce City runs above the Colorado county average on diagnosed diabetes. The chronic-condition Special Needs Plans on the Adams County list are built around: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus. If you live with one of those, a C-SNP may be worth a look; you need a qualifying diagnosis to join.
Medicare-certified hospitals in Brighton and Thornton from the CMS Hospital General Information file, with the CMS overall star rating. A rating is not network status: check each hospital against a plan.
2 Medicare-certified hospitals are listed; whether each is in a plan’s network is a separate question.

Your plan mailed an Annual Notice of Change by September 30, 2026. It lists what changes on January 1, 2027: premium, deductible, out-of-pocket limit, copays, drug list and network. Compare it line by line, or bring it to Lisa.
Counting plans with drug coverage, Special Needs Plans included, the Adams County list goes from 64 plans in 2026 to 59 in 2027, and the average in-network out-of-pocket limit moves from $6,497 to $7,199.

These counts cover plans with drug coverage, Special Needs Plans included, so they differ from the headline count of 36, which leaves out Special Needs Plans and includes plans without drug coverage. Clear Spring Health has no Medicare Advantage plan with drug coverage in Adams County for 2027.
8 plans on the 2026 Adams County list end with no similar plan in the county, including Select Health Medicare + Kroger (HMO), Select Health Medicare Flex (HMO), Select Health Medicare Active (HMO). 4 more plans end while the same company offers a plan with the same or a similar name under a new plan ID (HealthSpring TotalCare (HMO D-SNP), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring Preferred (HMO) and Aetna Medicare Signature (HMO-POS)). Members are not moved into the new plan automatically; to keep it, enroll in the new plan ID by December 7, 2026. The full list is on the Adams County guide.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.
New to terms like MOOP, HMO-POS or D-SNP? They are explained in plain English on the Adams County page and in the guide Medicare Advantage plan types.
Commerce City uses the Adams County plan list. It has 36 Medicare Advantage plans for 2027 (Special Needs Plans not included) from 10 organizations. Of those, 33 have a $0 Part C premium, and 30 have a $0 combined monthly premium for Parts C and D (25 of those include drug coverage; 5 have no drug coverage, so no Part D premium). You still pay the Part B premium, which is $202.90 a month in 2026; the 2027 amount is announced around November 2026. Source: CMS 2027 Landscape file.
You can keep Original Medicare with Health First Colorado, or look at a Dual Special Needs Plan (D-SNP) on the Adams County list. A D-SNP coordinates both programs in one plan. Health First Colorado decides your Medicaid eligibility and level, and that decides which D-SNPs you can join, so Lisa starts with your Medicaid letter.
Colorado runs its own marketplace, Connect for Health Colorado, not HealthCare.gov. Open enrollment for 2027 coverage starts November 1, 2026. Financial help depends on household income, and people with low income may qualify for Health First Colorado instead.
Counting Medicare Advantage plans with drug coverage, Special Needs Plans included, Adams County goes from 64 plans in 2026 to 59 in 2027. (The headline count of 36 plans on this page leaves out Special Needs Plans and includes plans without drug coverage, which is why the numbers differ.) The CMS crosswalk shows 8 plans ending with no similar plan in the county, 4 plans ending while the same company offers a plan with the same or a similar name under a new plan ID (members must enroll in it themselves), 3 plans folding into another plan automatically, and 10 new plans. Clear Spring Health has no Medicare Advantage plan with drug coverage in Adams County for 2027. Your plan’s Annual Notice of Change lists what changes for you on January 1.
Census figures (ACS 2020 to 2024 5-year estimates) show 7.4% of Commerce City residents with no health insurance, against 7.7% for Colorado. People under 65 can shop for coverage on Connect for Health Colorado, the state’s own marketplace; open enrollment for 2027 coverage starts November 1, 2026.
