D-SNP: 18 plans
For people with both Medicare and Health First Colorado (Colorado Medicaid).
Douglas County has the lowest uninsured rate and the highest incomes of the nine counties in this guide. Here is every Medicare Advantage plan CMS lists for it in 2027.
Examples of plans with drug coverage and a $0 combined monthly premium for Parts C and D, by 2026 star rating and out-of-pocket limit
CMS Medicare Advantage and Part D Landscape, contract year 2027. A plan's fit depends on your doctors and prescriptions; Lisa checks both before you choose.
Every figure comes from the CMS Landscape file for contract year 2027. Special Needs Plans are counted separately.
Two ways to count "$0 premium" plans: 29 have a $0 Part C premium, and 26 have a $0 combined monthly premium for Parts C and D (21 of those include drug coverage; 5 have no drug coverage, so no Part D premium). The average premium figures likewise show the Part C premium and the combined Part C + D premium separately. Star ratings shown are 2026 ratings, because CMS’s 2027 ratings are not yet in our data snapshot.
PPO plans are the largest group here (13 of 32), and 5 plans have no drug coverage.
| Plan type | Plans | $0 Part C premium | Average Part C premium | Average out-of-pocket limit | Without drug coverage |
|---|---|---|---|---|---|
| PPO | 13 | 12 | $0.69 | $7,050 | 3 |
| HMO | 10 | 10 | $0 | $5,480 | 1 |
| HMO-POS | 9 | 7 | $26.89 | $4,783 | 1 |
Grouped by plan type, because type decides how the network works: HMOs expect you to stay in network, PPOs let you go outside it at a higher cost.
13 plans, 12 with a $0 Part C premium, average Part C premium $0.69, average out-of-pocket limit $7,050, 3 without drug coverage.

CMS rates each contract from 1 to 5 stars. This shows the 2026 rating for each of the 32 Douglas County plans, because CMS’s 2027 ratings are not yet in our data snapshot. New plans and some small contracts are not rated.
No plan on the Douglas County list has a 5-star 2026 rating.
The Census Bureau counts about 377,200 Douglas County residents, 13.9% of them 65 or older, with a median household income of about $149,600 and 3.9% without health insurance.
Four Douglas County hospitals appear in the CMS file, in Castle Rock, Highlands Ranch, Lone Tree and Parker. The county list also applies to the small Douglas County part of Aurora.

Douglas County government runs an Older Adult Initiative for residents 60 and older, covering help with transportation, in-home support, housing and meals. The general county line is 303-660-7400. Aging Resources of Douglas County answers the Resource Navigation line, 303-814-4300, and the county also funds partner agencies for chore, homemaker and personal care services.
For activities, the county points to the Seniors' Council of Douglas County and its Living Well/Aging Well program, the Castle Rock Senior Activity Center, the Parker Senior Center and the Highlands Ranch Metro District's active adult programs. To report suspected mistreatment or self-neglect of an at-risk adult, call 303-663-6270.
Start with the county's I Need a Ride program, which gives free information and referrals to transportation for older adults, people with disabilities and their families. You need a physical address in Douglas County, and grant-subsidized rides are open to residents who are 60 or older, live with a disability, or have income below 200 percent of poverty; meeting any one of those is enough. In Lone Tree and Highlands Ranch, Link On Demand, formerly Lone Tree Link, offers free on-demand rides. The county's ride page also lists RTD bus, FlexRide, light rail and Access-a-Ride, so ask Resource Navigation which option reaches your doctor from your address.
Rail reaches the north end of the county. RTD's Southeast Rail Extension added 2.3 miles from Lincoln Station to RidgeGate Parkway in Lone Tree, including a station at Sky Ridge Avenue near Sky Ridge Medical Center. Since June 7, 2026, the R Line runs all the way to RidgeGate Parkway, so a Lone Tree rider can reach Aurora's Fitzsimons Station, the stop for the Anschutz Medical Campus, on the R Line.
Transportation and networks go together here. Douglas County's hospitals in the CMS file are AdventHealth Castle Rock, AdventHealth Parker, UCHealth Highlands Ranch Hospital and Sky Ridge Medical Center in Lone Tree, and they do not all belong to one health system. Before choosing a 2027 plan, check that the hospital you can actually get to is in its network.
Douglas County Human Services takes applications for Medicare Savings Programs, which help people with limited income and resources pay some or all of their Medicare premiums, and in some cases certain deductibles and coinsurance. You must have Medicare or be eligible for it, and monthly income and resource limits apply. Apply online through Colorado PEAK, call the state at 1-800-221-3943, or file a paper application with the county; mailed applications go to 4400 Castleton Court, Castle Rock, CO 80109. The Human Services office is at 11045 East Lansing Circle in Englewood (Meridian), phone 303-688-4825.
Qualifying for the Medicare Savings Program at the QMB, SLMB or QI level also brings Extra Help with prescription drug costs, according to Medicare.gov, so it is worth applying before you choose a Part D or Medicare Advantage plan rather than after.
The same office handles Health First Colorado and Old Age Pension. Douglas County lists the Old Age Pension rules as age 60 or older, income below $1,032 a month, and resources under $2,000 for an individual or $3,000 for a married couple, with benefits paid on an EBT card. Case questions go to 303-688-4825, Monday through Friday from 8 a.m. to 5 p.m. For free, unbiased Medicare counseling, the Denver Regional Council of Governments' State Health Insurance Assistance Program serves Douglas County. When you are ready to compare and enroll, Lisa works with Douglas County clients by phone or video.

Medicare-certified hospitals the CMS Hospital General Information file records in Douglas County, with the CMS overall star rating. A rating is not network status: check each hospital against a plan.
4 Medicare-certified hospitals are listed, 3 with a 5 of 5 CMS overall rating; whether each is in a plan’s network is a separate question.

Your plan mailed an Annual Notice of Change by September 30, 2026. It lists what changes on January 1, 2027: premium, deductible, out-of-pocket limit, copays, drug list and network. Compare it line by line, or bring it to Lisa.
Counting plans with drug coverage, Special Needs Plans included, the Douglas County list goes from 62 plans in 2026 to 54 in 2027, and the average in-network out-of-pocket limit moves from $6,220 to $7,066.
| Measure | 2026 | 2027 |

New to the Douglas County list for 2027: UHC Dual Complete CO-S4 (HMO-POS D-SNP), HealthSpring Preferred (HMO), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring TotalCare (HMO D-SNP), Perennial Advantage Strive (HMO I-SNP), Perennial Advantage Premier (HMO-POS I-SNP), Anthem Chronic Care (HMO-POS C-SNP), Humana Total Complete (HMO), Aetna Medicare Full Dual Extra Care (HMO D-SNP), Aetna Medicare Signature (HMO), Aetna Medicare Prime (HMO), DEVOTED C-SNP GIVEBACK EXTRAS 016 CO (HMO C-SNP), DEVOTED GIVEBACK EXTRAS 019 CO (HMO). Some of these carry the same or a similar name as a 2026 plan that ends (see the table above); they are new plan IDs, so members of the old plan must enroll in them.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.
Plan choice follows the conditions you actually live with, because they decide which specialists and drugs you need covered.

An SNP is a Medicare Advantage plan limited to people who meet a condition. They are left out of the counts above because not everyone can join.
For people with both Medicare and Health First Colorado (Colorado Medicaid).
For people with qualifying chronic conditions: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus.
For people who live in, or need the level of care of, a long-term care facility.

Every Advantage plan on this page replaces Original Medicare with a plan network. The other path keeps Original Medicare and adds a Medicare Supplement (Medigap) policy plus a separate Part D plan.
Medigap has no network: any provider that accepts Medicare accepts it, anywhere in the country. Premiums are higher than most Advantage plans, and in exchange your costs are more predictable. The Colorado Division of Insurance publishes Medigap premium charts for every company selling in the state, so you can compare the same plan letter across insurers.
Timing matters. Your 6-month Medigap open enrollment window starts when you are 65 or older and enrolled in Part B. Inside it, an insurer cannot turn you down or charge more for your health. Outside it, in most cases it can. See Medigap rules in Colorado and Medicare Advantage vs Medigap.
You must have Medicare Part A and Part B to join a Medicare Advantage plan, and live in the plan's service area, here Douglas County.
Seven months around your 65th birthday: the 3 months before, your birthday month, and the 3 months after.
October 15 to December 7, 2026. Choose or change plans for 2027; coverage starts January 1.
January 1 to March 31. If you are in a Medicare Advantage plan, you can switch plans or return to Original Medicare once.
Moving, losing employer coverage, gaining or losing Health First Colorado, or a plan not renewing can open a window outside the normal calendar.
Ways to enroll: with a licensed agent such as Lisa ((303) 887-8584), directly with a plan, or on Medicare.gov. Free counseling is also available from the Colorado SHIP at 888-696-7213. More detail: Medicare enrollment periods.
Douglas County has 32 Medicare Advantage plans for 2027 (Special Needs Plans not included) from 7 organizations. Of those, 29 have a $0 Part C premium, and 26 have a $0 combined monthly premium for Parts C and D (21 of those include drug coverage; 5 have no drug coverage, so no Part D premium). You still pay the Part B premium, which is $202.90 a month in 2026; the 2027 amount is announced around November 2026. Source: CMS 2027 Landscape file.
In the CMS Hospital General Information file, AdventHealth Castle Rock, UCHealth Highlands Ranch Hospital and AdventHealth Parker carry a 5 out of 5 overall rating; Sky Ridge Medical Center in Lone Tree is rated 3.
Yes, though they overlap. CMS approves plans county by county, so a Douglas County address always uses the Douglas County list.
Counting Medicare Advantage plans with drug coverage, Special Needs Plans included, Douglas County goes from 62 plans in 2026 to 54 in 2027. (The headline count of 32 plans on this page leaves out Special Needs Plans and includes plans without drug coverage, which is why the numbers differ.) The CMS crosswalk shows 13 plans ending with no similar plan in the county, 5 plans ending while the same company offers a plan with the same or a similar name under a new plan ID (members must enroll in it themselves), 3 plans folding into another plan automatically, and 13 new plans. Clear Spring Health has no Medicare Advantage plan with drug coverage in Douglas County for 2027; Perennial Advantage is new to the Douglas County list. Your plan’s Annual Notice of Change lists what changes for you on January 1.
Census figures (ACS 2020 to 2024 5-year estimates) show 3.9% of Douglas County residents with no health insurance, against 7.7% for Colorado. People under 65 can shop for coverage on Connect for Health Colorado, the state’s own marketplace; open enrollment for 2027 coverage starts November 1, 2026.
