My Fort Collins plan is changing for 2027. What should I do?
Read your Annual Notice of Change first, then compare. In the CMS Medicare Advantage landscape files and the CMS plan crosswalk, Larimer County has 41 Medicare Advantage plans with drug coverage for 2027, Special Needs Plans included, down from 50 in 2026. Of the 2027 plans, 27 have a $0 premium, and Clear Spring Health is leaving the county.
The Annual Enrollment Period runs from October 15 to December 7, 2026, and a change you make then starts January 1, 2027. If your plan is ending or merging, Lisa lays the remaining Larimer County plans side by side against your doctors and prescriptions, free and with no obligation, by phone or video, and handles the enrollment. You can also compare plans on Medicare.gov or call 1-800-MEDICARE (1-800-633-4227).
Will switching plans change which Fort Collins hospital I can use?
It can. Fort Collins has hospitals from two different health systems in the CMS file: UCHealth Poudre Valley Hospital at 1024 South Lemay Avenue, and Banner Fort Collins Medical Center at 4700 Lady Moon Drive. In Loveland, UCHealth Medical Center of the Rockies and a Banner North Colorado Medical Center campus split the same way.
Because networks tend to follow systems, moving from one Medicare Advantage plan to another can quietly move you from one system to the other. Before you pick a replacement plan, list:
- Your primary care doctor and the system their clinic belongs to.
- Every specialist you saw this year, and where they admit patients.
- The hospital you would want for a planned surgery, and the one nearest home for an emergency.
What if I can no longer drive to appointments in Fort Collins?
Transfort, the city bus system, runs Dial-A-Ride, an ADA paratransit service for people whose disability prevents them from using the fixed-route buses some or all of the time. Age alone does not qualify you: you apply and are certified, which can take up to 21 calendar days. Service covers trips that start and end within three quarters of a mile of a fixed bus route, Monday to Saturday from 6 a.m. to 9:30 p.m., and 8 a.m. to 7 p.m. on holidays Reservations and questions go to 970-224-6066, and the eligibility specialist is at 970-224-6002. Dial-A-Ride riders can also use Dial-A-Taxi, with a voucher worth up to $15 per trip. Visiting family who hold ADA paratransit certification from another U.S. city can ride for up to 21 days a year.
If rides are part of how you get care, mention it when you compare plans. Some Medicare Advantage plans include a transportation benefit and some do not, and its rules differ from Dial-A-Ride’s.
Does my plan’s fitness benefit work at the Fort Collins Senior Center?
Ask before you assume. The Fort Collins Senior Center at 1200 Raintree Drive is open to all but specializes in programming for adults 50 and older, with a 25-yard lap pool and hot tub, a gym, an indoor track, classrooms, a woodworking shop and a library (970-221-6644). Many people choose a Medicare Advantage plan partly for a fitness benefit, but participating locations differ by plan. Lisa can check whether a plan you like works at the facility you actually use, so the benefit is real for you and not just a line in a brochure.

Larimer County plans: 2026 to 2027
Your plan mailed an Annual Notice of Change by September 30, 2026. It lists what changes on January 1, 2027: premium, deductible, out-of-pocket limit, copays, drug list and network. Compare it line by line, or bring it to Lisa.
Counting plans with drug coverage, Special Needs Plans included, the Larimer County list goes from 50 plans in 2026 to 41 in 2027, and the average in-network out-of-pocket limit moves from $6,882 to $7,753.
| Measure | 2026 | 2027 |
|---|---|---|
| Plans with drug coverage (SNPs included) | 50 | 41 |
| With a $0 combined monthly premium for Parts C and D | 26 | 27 |
| Average in-network out-of-pocket limit | $6,882 | $7,753 |
| Average Part D deductible | $438.60 | $542.56 |
| Maximum Part D deductible allowed by CMS | $615 | $700 |
| Part D out-of-pocket cap | $2,100 | $2,400 |
These counts cover plans with drug coverage, Special Needs Plans included, so they differ from the headline count of 24, which leaves out Special Needs Plans and includes plans without drug coverage. Clear Spring Health has no Medicare Advantage plan with drug coverage in Larimer County for 2027.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.

How the figures are derived
- Headline plan counts and averages exclude Special Needs Plans, which have their own eligibility rules and are reported separately.
- "$0 Part C premium" counts plans whose Part C premium is $0. "$0 combined monthly premium for Parts C and D" counts plans whose consolidated Part C + D premium is $0 (for a plan without drug coverage, its Part C premium); our guides and blog use this second count. Out-of-pocket limits are in-network amounts. The average Part D deductible covers plans with drug coverage only.
- The 2026 to 2027 comparison counts Medicare Advantage plans with drug coverage, Special Needs Plans included, and its "$0" row uses the combined Part C + D premium, so it differs from the headline count.
- When the CMS crosswalk ends a plan with no successor but the same company files a plan with the same or a similar name under a new plan ID, we say so; members are not moved into the new ID automatically.
- We do not publish per-plan enrollment: the source does not carry it, and we never estimate plan figures.
