D-SNP: 19 plans
For people with both Medicare and Health First Colorado (Colorado Medicaid).
Arapahoe County covers most of Aurora and the southern suburbs. This guide shows every Medicare Advantage plan CMS lists for the county in 2027.
Examples of plans with drug coverage and a $0 combined monthly premium for Parts C and D, by 2026 star rating and out-of-pocket limit
CMS Medicare Advantage and Part D Landscape, contract year 2027. A plan's fit depends on your doctors and prescriptions; Lisa checks both before you choose.
Every figure comes from the CMS Landscape file for contract year 2027. Special Needs Plans are counted separately.
Two ways to count "$0 premium" plans: 30 have a $0 Part C premium, and 27 have a $0 combined monthly premium for Parts C and D (22 of those include drug coverage; 5 have no drug coverage, so no Part D premium). The average premium figures likewise show the Part C premium and the combined Part C + D premium separately. Star ratings shown are 2026 ratings, because CMS’s 2027 ratings are not yet in our data snapshot.
PPO plans are the largest group here (13 of 33), and 5 plans have no drug coverage.
| Plan type | Plans | $0 Part C premium | Average Part C premium | Average out-of-pocket limit | Without drug coverage |
|---|---|---|---|---|---|
| PPO | 13 | 12 | $0.69 | $7,050 | 3 |
| HMO | 10 | 10 | $0 | $6,000 | 1 |
| HMO-POS | 10 | 8 | $24.20 | $4,655 | 1 |
Grouped by plan type, because type decides how the network works: HMOs expect you to stay in network, PPOs let you go outside it at a higher cost.
13 plans, 12 with a $0 Part C premium, average Part C premium $0.69, average out-of-pocket limit $7,050, 3 without drug coverage.

An SNP is a Medicare Advantage plan limited to people who meet a condition. They are left out of the counts above because not everyone can join.
For people with both Medicare and Health First Colorado (Colorado Medicaid).
For people with qualifying chronic conditions: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus.
For people who live in, or need the level of care of, a long-term care facility.
19 plans, with monthly Part C + D premiums from $0 to $18.40. You must meet the plan’s condition to join.

The Census Bureau counts about 659,800 Arapahoe County residents, roughly 95,100 of them 65 or older, with a median household income of about $101,100. 9.3% of residents have no health insurance.
The Arapahoe County list applies to most Aurora addresses and the rest of the county. The tables below group plans by type, and the Special Needs Plans section shows the plans for people with both Medicare and Health First Colorado.
How Lisa helps here. Arapahoe County clients work with Lisa by phone or video, or at the Brighton office.
14.4% of Arapahoe County residents are 65 or older, against 15.6% for Colorado, and 9.3% have no health insurance.

Arapahoe County Human Services runs the county side of the state's adult programs. Its list for older adults includes the Medicare Savings Program, which helps with Medicare premiums and related costs; Medicaid, called Health First Colorado; Old Age Pension for adults 60 and older with limited income; Long-Term Care for people who need help with daily living at home or in a facility; LEAP heating help, SNAP food assistance and burial assistance.
You can apply for Health First Colorado online through Colorado PEAK, by phone at 1-800-221-3943, or in person at the Human Services offices in Aurora or Littleton (5334 South Prince Street). The county's main line is 303-795-4400, and its Adult Protection hotline, 303-636-1750, takes confidential reports of abuse, neglect or exploitation around the clock.
The Medicare Savings Program is worth applying for even if you do not expect to qualify for Medicaid. Medicare.gov says people who qualify at the QMB, SLMB or QI level also get Extra Help paying for prescription drugs, which can change which Part D plan or Medicare Advantage plan costs you least over the year.
If you end up with both Medicare and Health First Colorado, look at the Special Needs Plans section of this page. Those plans are built for people with both kinds of coverage, and choosing among them is a network and drug list decision like any other.
The Denver Regional Council of Governments (DRCOG) is the Area Agency on Aging for Arapahoe County, and its State Health Insurance Assistance Program serves Arapahoe, Douglas and Jefferson counties. Its counselors give free one-on-one help to people on Medicare and their families, explaining benefits, coverage rules and appeal rights across Original Medicare, Medicare Advantage, Part D and Medigap. DRCOG's general information line is 303-480-6700, and Colorado's statewide SHIP line is 1-888-696-7213.
SHIP counselors and a broker do different jobs. SHIP explains your options without enrolling you in a plan; Lisa compares the 2027 plans she is contracted to offer against your doctors and drugs, then handles the enrollment and stays with you for later questions. You can use both.
Arapahoe County says it has more than 100,000 residents over the age of 60, and its Community Resources department runs a Senior Resources program for them. A homemaker program provides free light housekeeping for older adults and people with a disability who meet income limits, and chore services cover seasonal yard work, outdoor first-floor window washing and larger cleaning projects. Call 303-738-8080.
For rides, the county points residents to Via Mobility Services, which uses accessible vehicles for trips to medical appointments, dining centers, grocery stores and food banks (303-447-2848; Spanish 303-444-3043). Public health is also a county service now: Arapahoe County Public Health opened on January 1, 2023, after the Tri-County Health Department served the county from 1948 through 2022.
Yes, through Colorado's senior property tax exemption, which the Arapahoe County Assessor administers. At least one owner must be 65 or older as of January 1 and must have occupied the home as a primary residence for at least 10 consecutive years. Qualifying seniors have 50% of the first $200,000 of the home's actual value exempted from property tax. Applications are due on or before July 15 for that tax year.
The exemption has nothing to do with Medicare eligibility, but it is money back in a retiree's budget. When you weigh a higher-premium Medigap plan against a lower-premium Medicare Advantage plan, knowing your fixed costs, including property tax, makes the choice clearer.

CMS rates each contract from 1 to 5 stars. This shows the 2026 rating for each of the 33 Arapahoe County plans, because CMS’s 2027 ratings are not yet in our data snapshot. New plans and some small contracts are not rated.
No plan on the Arapahoe County list has a 5-star 2026 rating.
Your plan mailed an Annual Notice of Change by September 30, 2026. It lists what changes on January 1, 2027: premium, deductible, out-of-pocket limit, copays, drug list and network. Compare it line by line, or bring it to Lisa.
Counting plans with drug coverage, Special Needs Plans included, the Arapahoe County list goes from 61 plans in 2026 to 56 in 2027, and the average in-network out-of-pocket limit moves from $6,472 to $7,145.
| Measure | 2026 | 2027 |
|---|---|---|
| Plans with drug coverage (SNPs included) | 61 | 56 |
| With a $0 combined monthly premium for Parts C and D | 35 | 40 |
| Average in-network out-of-pocket limit | $6,472 | $7,145 |
| Average Part D deductible | $400.66 | $499.70 |
| Maximum Part D deductible allowed by CMS | $615 | $700 |
| Part D out-of-pocket cap | $2,100 | $2,400 |
These counts cover plans with drug coverage, Special Needs Plans included, so they differ from the headline count of 33, which leaves out Special Needs Plans and includes plans without drug coverage. Clear Spring Health has no Medicare Advantage plan with drug coverage in Arapahoe County for 2027.

New to the Arapahoe County list for 2027: UHC Dual Complete CO-S4 (HMO-POS D-SNP), HealthSpring Preferred (HMO), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring TotalCare (HMO D-SNP), Anthem Chronic Care (HMO-POS C-SNP), Aetna Medicare Full Dual Extra Care (HMO D-SNP), Aetna Medicare Signature (HMO), Aetna Medicare Prime (HMO), DEVOTED C-SNP GIVEBACK EXTRAS 016 CO (HMO C-SNP), DEVOTED GIVEBACK EXTRAS 019 CO (HMO). Some of these carry the same or a similar name as a 2026 plan that ends (see the table above); they are new plan IDs, so members of the old plan must enroll in them.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.
Plan choice follows the conditions you actually live with, because they decide which specialists and drugs you need covered.

CDC PLACES: Local Data for Better Health, County Data, 2025 release (2023 survey estimates), retrieved 2026-10-14. Model-based small-area estimates of crude prevalence among adults 18 and over, not a count of local diagnoses. Crude rates follow the age mix of a place: an older community reads higher on most conditions. Colorado figure: average of the 64 Colorado counties, weighted by adult population.
Arapahoe County runs above the Colorado county average on high blood pressure. The chronic-condition Special Needs Plans on the Arapahoe County list are built around: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus. If you live with one of those, a C-SNP may be worth a look; you need a qualifying diagnosis to join.
Medicare-certified hospitals the CMS Hospital General Information file records in Arapahoe County, with the CMS overall star rating. A rating is not network status: check each hospital against a plan.
3 Medicare-certified hospitals are listed; whether each is in a plan’s network is a separate question.

Colorado runs its own marketplace for individual and family health insurance: Connect for Health Colorado, not HealthCare.gov. Open enrollment for 2027 coverage starts November 1, 2026. Financial help depends on household income, and people with low income may qualify for Health First Colorado, the state Medicaid program, or Child Health Plan Plus (CHP+) for children.
CDC estimates that 11.5% of adults aged 18 to 64 in Arapahoe County have no health insurance (2023 estimates), against a Colorado county average of 11.1%.
Lisa helps with Connect for Health Colorado plans and pre-65 coverage too. Try the ACA subsidy calculator, read the Connect for Health Colorado guide, or see individual and family health insurance. Estimates are estimates; final premiums are set at enrollment.
Headwaters Health and Wealth is not affiliated with, endorsed by, or operating on behalf of Connect for Health Colorado, HealthCare.gov, the Health Insurance Marketplace, or any federal or state government agency. Plan availability, premiums and advance premium tax credits are set by the carriers and the Marketplace.
Every Advantage plan on this page replaces Original Medicare with a plan network. The other path keeps Original Medicare and adds a Medicare Supplement (Medigap) policy plus a separate Part D plan.
Medigap has no network: any provider that accepts Medicare accepts it, anywhere in the country. Premiums are higher than most Advantage plans, and in exchange your costs are more predictable. The Colorado Division of Insurance publishes Medigap premium charts for every company selling in the state, so you can compare the same plan letter across insurers.
Timing matters. Your 6-month Medigap open enrollment window starts when you are 65 or older and enrolled in Part B. Inside it, an insurer cannot turn you down or charge more for your health. Outside it, in most cases it can. See Medigap rules in Colorado and Medicare Advantage vs Medigap.
You must have Medicare Part A and Part B to join a Medicare Advantage plan, and live in the plan's service area, here Arapahoe County.
Seven months around your 65th birthday: the 3 months before, your birthday month, and the 3 months after.
October 15 to December 7, 2026. Choose or change plans for 2027; coverage starts January 1.
January 1 to March 31. If you are in a Medicare Advantage plan, you can switch plans or return to Original Medicare once.
Moving, losing employer coverage, gaining or losing Health First Colorado, or a plan not renewing can open a window outside the normal calendar.
Ways to enroll: with a licensed agent such as Lisa ((303) 887-8584), directly with a plan, or on Medicare.gov. Free counseling is also available from the Colorado SHIP at 888-696-7213. More detail: Medicare enrollment periods.

Counting Medicare Advantage plans with drug coverage, Special Needs Plans included, Arapahoe County goes from 61 plans in 2026 to 56 in 2027.
