D-SNP: 19 plans
For people with both Medicare and Health First Colorado (Colorado Medicaid).
Adams County is home base: the office is in Brighton, the county seat. This guide lays out every Medicare Advantage plan CMS lists for the county in 2027, with the numbers that decide which ones deserve a closer look.
Examples of plans with drug coverage and a $0 combined monthly premium for Parts C and D, by 2026 star rating and out-of-pocket limit
CMS Medicare Advantage and Part D Landscape, contract year 2027. A plan's fit depends on your doctors and prescriptions; Lisa checks both before you choose.
Every figure comes from the CMS Landscape file for contract year 2027. Special Needs Plans are counted separately.
Two ways to count "$0 premium" plans: 33 have a $0 Part C premium, and 30 have a $0 combined monthly premium for Parts C and D (25 of those include drug coverage; 5 have no drug coverage, so no Part D premium). The average premium figures likewise show the Part C premium and the combined Part C + D premium separately. Star ratings shown are 2026 ratings, because CMS’s 2027 ratings are not yet in our data snapshot.
PPO plans are the largest group here (14 of 36), and 5 plans have no drug coverage.
| Plan type | Plans | $0 Part C premium | Average Part C premium | Average out-of-pocket limit | Without drug coverage |
|---|---|---|---|---|---|
| PPO | 14 | 13 | $0.64 | $7,057 | 3 |
| HMO-POS | 12 | 10 | $20.17 | $5,192 | 1 |
| HMO | 10 | 10 | $0 | $6,000 | 1 |
Grouped by plan type, because type decides how the network works: HMOs expect you to stay in network, PPOs let you go outside it at a higher cost.
14 plans, 13 with a $0 Part C premium, average Part C premium $0.64, average out-of-pocket limit $7,057, 3 without drug coverage.

CMS rates each contract from 1 to 5 stars. This shows the 2026 rating for each of the 36 Adams County plans, because CMS’s 2027 ratings are not yet in our data snapshot. New plans and some small contracts are not rated.
No plan on the Adams County list has a 5-star 2026 rating.
Your plan mailed an Annual Notice of Change by September 30, 2026. It lists what changes on January 1, 2027: premium, deductible, out-of-pocket limit, copays, drug list and network. Compare it line by line, or bring it to Lisa.
Counting plans with drug coverage, Special Needs Plans included, the Adams County list goes from 64 plans in 2026 to 59 in 2027, and the average in-network out-of-pocket limit moves from $6,497 to $7,199.
| Measure | 2026 | 2027 |
|---|---|---|
| Plans with drug coverage (SNPs included) | 64 | 59 |
| With a $0 combined monthly premium for Parts C and D | 38 | 43 |
| Average in-network out-of-pocket limit | $6,497 | $7,199 |
| Average Part D deductible | $404.69 | $510.90 |
| Maximum Part D deductible allowed by CMS | $615 | $700 |
| Part D out-of-pocket cap | $2,100 | $2,400 |
These counts cover plans with drug coverage, Special Needs Plans included, so they differ from the headline count of 36, which leaves out Special Needs Plans and includes plans without drug coverage. Clear Spring Health has no Medicare Advantage plan with drug coverage in Adams County for 2027.

New to the Adams County list for 2027: UHC Dual Complete CO-S4 (HMO-POS D-SNP), HealthSpring Preferred (HMO), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring TotalCare (HMO D-SNP), Aetna Medicare Chronic Care (HMO C-SNP), Aetna Medicare Full Dual Extra Care (HMO D-SNP), Aetna Medicare Signature (HMO), Aetna Medicare Prime (HMO), DEVOTED C-SNP GIVEBACK EXTRAS 016 CO (HMO C-SNP), DEVOTED GIVEBACK EXTRAS 019 CO (HMO). Some of these carry the same or a similar name as a 2026 plan that ends (see the table above); they are new plan IDs, so members of the old plan must enroll in them.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Confirm your dates on Medicare.gov. The official Medicare & You handbook explains each option.
The Census Bureau counts about 530,200 Adams County residents, and about 59,900 of them, 11.3%, are 65 or older, a younger profile than Colorado as a whole (15.6%). 11.6% of residents have no health insurance, against 7.7% statewide.
Medicare Advantage plans are approved county by county. If you live anywhere in Adams County, from Brighton and Henderson to Thornton, Commerce City and the northern part of Aurora, the plans below are the ones CMS lists for your address in 2027. Whether a plan suits you depends on your doctors, your prescriptions and the hospitals you use, so that is where every review starts.
How Lisa helps here. Lisa Asmussen meets Adams County clients at the Brighton office and by phone or video. Her focus is education: you leave knowing what your plan covers, how to use it and what it costs, and she is there for the yearly review.
11.3% of Adams County residents are 65 or older, against 15.6% for Colorado, and 11.6% have no health insurance.

Adams County covers 1,166.3 square miles and borders Arapahoe, Washington, Morgan, Weld, Jefferson, Broomfield and Denver counties. According to the county, it includes the cities of Brighton, Commerce City, Federal Heights, Northglenn and Thornton, parts of Arvada, Aurora, Lochbuie and Westminster, the Town of Bennett, and the unincorporated communities of Henderson, Strasburg and Watkins.
Voters created the county in 1902 out of what had been a much larger Arapahoe County, and it was named for Alva Adams, a former governor. Brighton became the permanent county seat in a November 8, 1904 election, and the 1906 courthouse at 4th and Bridge Street now serves as Brighton City Hall.
That whole range, from Federal Heights on the west side to Strasburg and Watkins far to the east, sits under one county plan list. A plan that fits Federal Heights may be a poor fit out east, where in-network doctors can be a long drive away. If you live in the eastern half of the county, look closely at where a plan's network doctors actually are, and at whether a PPO's out-of-network coverage is worth the higher cost.
The Denver Regional Council of Governments (DRCOG) is the Area Agency on Aging for Adams, Arapahoe, Broomfield, Clear Creek, Denver, Douglas, Jefferson and Gilpin counties. Through DRCOG you can get Medicare help by phone, email or in person, request a ride to the doctor, and reach a long-term care ombudsman if a relative lives in a nursing home or assisted living residence.
Colorado's State Health Insurance Assistance Program (SHIP), run by the Division of Insurance, takes calls statewide at 888-696-7213. Inside the county, the city senior centers add local options: the Sue Corbett Active Adult Center at Eagle View in Brighton hosts a SHIP counselor monthly, and the Thornton Active Adult Center offers free Medicare 101 classes and counseling with Benefits in Action counselors by appointment.
Counselors explain the rules without selling anything. Lisa Asmussen is a licensed broker who can compare the plans she is contracted with and enroll you, and she is glad to work alongside a SHIP counselor if you have already spoken with one.
Adams County Human Services runs the Health First Colorado program from the Pete Mirelez Human Services Center at 11860 Pecos Street in Westminster, open weekdays from 7:30 a.m. to 5 p.m. (720-523-2000 for the department, 720-523-2700 for the main location). There is also a Brighton Workforce and Business Center at 36 South 18th Avenue, and the department lists a Veterans Office among its services.
The county resource guide lists more help: Benefits in Action keeps hours in the Community Partners Suite at Human Services, Connect for Health Colorado takes calls at 855-752-6749 for anyone under 65, and the guide names community health centers including Salud Family Health (Brighton and Commerce City), Clinica Family Health Services (Thornton and Westminster) and Metro Community Provider Network (Aurora).
If you have both Medicare and Health First Colorado, the Dual Special Needs Plans on the county list coordinate the two. If you qualify for a Medicare Savings Program, the state can pay your Part B premium. Lisa looks at those programs first, before premiums or extras.
In the CMS Hospital General Information file, four hospitals carry an Adams County record: University of Colorado Hospital at 12605 East 16th Avenue and VA Eastern Colorado Healthcare System at 1700 North Wheeling Street, both in the Adams County part of Aurora, Intermountain Health Platte Valley Hospital at 1600 Prairie Center Parkway in Brighton, and HCA HealthOne Mountain Ridge at 9191 Grant Street in Thornton.
Those four belong to different health systems, and Medicare Advantage networks are set plan by plan. Before you enroll, Lisa checks the hospital you would actually go to in an emergency, and the one your specialists use, against each plan for 2027. She meets Adams County clients at the Brighton office or by phone or video.

Colorado totals are the sum of all 64 Colorado counties in the same Census release; a statewide median is not computed here. Survey estimates for small towns carry wide margins of error.
Plan choice follows the conditions you actually live with, because they decide which specialists and drugs you need covered.
Compared with the Colorado county average, Adams County reads higher on diagnosed diabetes, high blood pressure, obesity and adults under 65 without insurance.

CDC PLACES: Local Data for Better Health, County Data, 2025 release (2023 survey estimates), retrieved 2026-10-14. Model-based small-area estimates of crude prevalence among adults 18 and over, not a count of local diagnoses. Crude rates follow the age mix of a place: an older community reads higher on most conditions. Colorado figure: average of the 64 Colorado counties, weighted by adult population.
Adams County runs above the Colorado county average on diagnosed diabetes and high blood pressure. The chronic-condition Special Needs Plans on the Adams County list are built around: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus. If you live with one of those, a C-SNP may be worth a look; you need a qualifying diagnosis to join.
An SNP is a Medicare Advantage plan limited to people who meet a condition. They are left out of the counts above because not everyone can join.
For people with both Medicare and Health First Colorado (Colorado Medicaid).
For people with qualifying chronic conditions: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus.
For people who live in, or need the level of care of, a long-term care facility.
19 plans, with monthly Part C + D premiums from $0 to $18.40. You must meet the plan’s condition to join.

Every Advantage plan on this page replaces Original Medicare with a plan network. The other path keeps Original Medicare and adds a Medicare Supplement (Medigap) policy plus a separate Part D plan.
Medigap has no network: any provider that accepts Medicare accepts it, anywhere in the country. Premiums are higher than most Advantage plans, and in exchange your costs are more predictable. The Colorado Division of Insurance publishes Medigap premium charts for every company selling in the state, so you can compare the same plan letter across insurers.
Timing matters. Your 6-month Medigap open enrollment window starts when you are 65 or older and enrolled in Part B. Inside it, an insurer cannot turn you down or charge more for your health. Outside it, in most cases it can. See Medigap rules in Colorado and Medicare Advantage vs Medigap.
Colorado runs its own marketplace for individual and family health insurance: Connect for Health Colorado, not HealthCare.gov. Open enrollment for 2027 coverage starts November 1, 2026. Financial help depends on household income, and people with low income may qualify for Health First Colorado, the state Medicaid program, or Child Health Plan Plus (CHP+) for children.
CDC estimates that 17.2% of adults aged 18 to 64 in Adams County have no health insurance (2023 estimates), against a Colorado county average of 11.1%.
Lisa helps with Connect for Health Colorado plans and pre-65 coverage too. Try the ACA subsidy calculator, read the Connect for Health Colorado guide, or see individual and family health insurance. Estimates are estimates; final premiums are set at enrollment.
Headwaters Health and Wealth is not affiliated with, endorsed by, or operating on behalf of Connect for Health Colorado, HealthCare.gov, the Health Insurance Marketplace, or any federal or state government agency. Plan availability, premiums and advance premium tax credits are set by the carriers and the Marketplace.

Brighton (except a small Weld County part), Commerce City, Thornton, Northglenn, Federal Heights, Henderson, the Adams County part of Westminster and the northern, Adams County part of Aurora. The plan list follows the county on your address, not the name of your city.
Each plan has its own network. An HMO usually expects you to use its network hospitals, while a PPO lets you go out of network at a higher cost. Before you enroll, check that the hospitals you would actually use, in Adams County or across the metro, are in the plan network for 2027.
