D-SNP: 16 plans
For people with both Medicare and Health First Colorado (Colorado Medicaid).
Greeley is the Weld County seat and its largest city. It also runs above the Colorado average on most chronic conditions, which makes the plan details more than a formality.
Examples of plans with drug coverage and a $0 combined monthly premium for Parts C and D, by 2026 star rating and out-of-pocket limit
CMS Medicare Advantage and Part D Landscape, contract year 2027. A plan's fit depends on your doctors and prescriptions; Lisa checks both before you choose.

How Lisa helps here. Greeley clients work with Lisa by phone or video, or at the Brighton office. She looks at the conditions you live with, the specialists you see and the drugs you take, then compares plans on those.

The Weld County Area Agency on Aging works out of the Department of Human Services campus at 315 N. 11th Ave., Building A, in Greeley. The Aging and Disability Resources for Colorado line, (970) 400-6952, helps older adults and families find care and support services across the county. If you need Health First Colorado or a Medicare Savings Program, you apply through the Department of Human Services in your county of residence, which for Greeley is Weld County.
The plan list itself is shrinking for 2027. In the CMS Medicare Advantage landscape files and the CMS plan crosswalk, Weld County has 42 Medicare Advantage plans with drug coverage for 2027, Special Needs Plans included, down from 53 in 2026, and Clear Spring Health is leaving the county. If your plan is one of those ending, your Annual Notice of Change will say so.
Where Lisa fits: she is a licensed broker, so she can show you plans side by side and enroll you, and she works with Greeley clients by phone or video. Her review is free with no obligation. You can also compare plans on Medicare.gov or call 1-800-MEDICARE (1-800-633-4227).
Yes, because the city’s two hospitals in the CMS file belong to two different health systems. Banner North Colorado Medical Center sits at 1801 16th Street near the center of town, and UCHealth Greeley Hospital sits at 6767 29th Street on the west side. Doctors, clinics and specialists tend to cluster around each system, so the hospital question usually decides the doctor question too.
Here is how it plays out by plan type:
Greeley Evans Transit (GET) offers reduced fares to riders 60 and over: $0.75 one way, or $92.00 for an annual pass that also covers persons with disabilities and Medicare card holders. To use the discount, you show a photo ID and proof of birth date, or your Medicare card, when you board. The discount card is issued at the GALLOP office, 101 11th Avenue, which also offers free photo IDs.
For riders who cannot use the bus because of a disability, GET runs ADA paratransit within three quarters of a mile of its fixed routes. Eligibility is decided by an application completed by you and your health care provider, trips are booked in advance by calling (970) 350-9290, and the fare is $3.00 per trip.
Why this belongs in a Medicare conversation: some Medicare Advantage plans include rides to medical appointments and some do not, and the details vary by plan. If you rely on rides to see your doctors, tell Lisa, and she will check the transportation benefit, if any, on each plan you compare, so you can weigh it against what GET already offers you.

Plan choice follows the conditions you actually live with, because they decide which specialists and drugs you need covered.

CDC PLACES: Local Data for Better Health, Place Data, 2025 release (2023 survey estimates), retrieved 2026-10-14. Model-based small-area estimates of crude prevalence among adults 18 and over, not a count of local diagnoses. Crude rates follow the age mix of a place: an older community reads higher on most conditions. Colorado figure: average of the 64 Colorado counties, weighted by adult population.
Of the conditions above where Greeley reads higher than the Colorado county average, diagnosed diabetes, high blood pressure, coronary heart disease and stroke are among those a chronic-condition Special Needs Plan can cover. The chronic-condition Special Needs Plans on the Weld County list are built around: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus. If you live with one of those, a C-SNP may be worth a look; you need a qualifying diagnosis to join.
Medicare Advantage plans are approved by county, so Greeley residents see the Weld County list. Figures from the CMS Landscape file for contract year 2027; Special Needs Plans are counted separately.
Two ways to count "$0 premium" plans: 23 have a $0 Part C premium, and 20 have a $0 combined monthly premium for Parts C and D (15 of those include drug coverage; 5 have no drug coverage, so no Part D premium). The average premium figures likewise show the Part C premium and the combined Part C + D premium separately. Star ratings shown are 2026 ratings: CMS published 2027 ratings on October 8, 2026, and the tables on this site still show each plan’s 2026 rating until its data snapshot is refreshed. Check a plan’s 2027 rating on Medicare.gov Plan Finder.

13 plans on the 2026 Weld County list end with no similar plan in the county, including Select Health Medicare + Kroger (HMO), Select Health Medicare Flex (HMO), Select Health Medicare Active (HMO). 4 more plans end while the same company offers a plan with the same or a similar name under a new plan ID (HealthSpring TotalCare (HMO D-SNP), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring Preferred (HMO) and Select Health Medicare Essential (HMO)). Members are not moved into the new plan automatically; to keep it, enroll in the new plan ID by December 7, 2026. The full list is on the Weld County guide.
If your plan is not renewing: you receive a non-renewal letter in October.
