Frederick at a glance
The Census Bureau counts about 16,700 Frederick residents, 11.5% of them 65 or older, with a median household income of about $129,500 and only 4.1% without health insurance. CDC PLACES estimates show Frederick adults below the Colorado county average on diabetes and heart disease, but above it on obesity (31.8%).
Frederick is in Weld County, so residents choose from the Weld County plan list when they reach Medicare. The HMO-POS plans are listed below: they keep most care in a network while allowing some out-of-network services at a higher cost. Before 65, coverage comes through an employer or Connect for Health Colorado. The nearest hospitals in the CMS file are in Longmont.

How did a coal camp become today's Frederick?
Frederick was incorporated in 1907 by three young women who named the town after their father, Frederick A. Clark, who owned the land. The town's own history describes mostly Italian immigrant families who came to work the area's coal mines for $2 a day, built the first Frederick High School in 1916, and banded together to buy the water source now known as Milavec Lake at the Frederick Recreation Area. The annual Miners Day celebration honors them.
The town now sits at the center of the Carbon Valley, alongside Firestone, Dacono, Mead and Erie, and describes itself as one of the fastest growing communities in Colorado. Growth like that brings two coverage stages, and this page covers both: family coverage before 65 and the move onto Medicare.
What should I know about the emergency room on Highway 52?
Frederick has a freestanding emergency and urgent care center: CommonSpirit Emergency and Urgent Care Indian Peaks, at 4943 State Highway 52. The town notes that Longmont United Hospital opened the first phase of the Indian Peaks Medical Center health campus, at Silver Birch Boulevard and Highway 52, in December 2013. The center says its ER runs 24 hours a day, and urgent care is available daily from 7 a.m. to 9 p.m.
Its billing disclosures are worth reading before you need them. Non-emergency cases seen during urgent care hours are billed at urgent care levels. Emergency patients receive two bills, one for the facility and one for the emergency physician services. The center accepts Medicare, Medicaid and CHP+, and it notes that the facility or a physician working there may not be in your insurance network.
Here is how that meets Medicare. Medicare Advantage HMOs cover emergencies even when the ER is outside their network, so a true emergency at Indian Peaks is covered. Routine follow-up care afterward is a different matter and usually needs a network provider. When you compare plans, look at both the emergency room copay and the urgent care copay, because the same building can bill either one.
Where can Frederick families get care and help enrolling?
Frederick also has a Salud Family Health clinic, at 5995 Iris Parkway, where medical, behavioral health and dental visits are offered on weekdays. Discounts on a sliding scale are available by income and household size.
For Frederick families under 65, that is the marketplace side: open enrollment for 2027 coverage on Connect for Health Colorado runs from November 1, 2026 through January 15, 2027, and enrolling by December 15 means coverage starts January 1. Financial help depends on household income. Lisa compares those plans with families by phone or video, and she can line up a parent's move onto Medicare at the same time.
Does my Medicare plan include the SilverSneakers classes in Frederick?
It might. The Carbon Valley Parks and Recreation District, which runs the Carbon Valley Recreation Center at 701 5th Street in Frederick, is a contracted SilverSneakers provider and offers SilverSneakers classes designed for older adults, along with chair yoga, balance and Tai Chi classes. The district also runs an Active Adults and Community Center for people 55 and older in neighboring Firestone.
SilverSneakers says its membership is available at no cost for adults 65 and older through select Medicare plans. That means it depends on the plan, not on Medicare itself, and participating locations can differ by plan. If you already go to classes at the rec center, add that to your list and ask for it to be checked on each plan you compare. It should not outweigh your doctors or prescriptions, but it is a fair tie breaker.

Members are not moved into the new plan automatically; to keep it, enroll in the new plan ID by December 7, 2026. The full list is on the Weld County guide.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. How the figures are derived
- Headline plan counts and averages exclude Special Needs Plans, which have their own eligibility rules and are reported separately.
- "$0 Part C premium" counts plans whose Part C premium is $0. "$0 combined monthly premium for Parts C and D" counts plans whose consolidated Part C + D premium is $0 (for a plan without drug coverage, its Part C premium); our guides and blog use this second count. Out-of-pocket limits are in-network amounts. The average Part D deductible covers plans with drug coverage only.
- The 2026 to 2027 comparison counts Medicare Advantage plans with drug coverage, Special Needs Plans included, and its "$0" row uses the combined Part C + D premium, so it differs from the headline count.
- When the CMS crosswalk ends a plan with no successor but the same company files a plan with the same or a similar name under a new plan ID, we say so; members are not moved into the new ID automatically.
- We do not publish per-plan enrollment: the source does not carry it, and we never estimate plan figures.
